Pharmacological action
Aceclofenac is a nonsteroidal anti-inflammatory drug, a derivative of phenylacetic acid. Aceclofenac has analgesic, anti-inflammatory, and antipyretic effects that are associated with inhibition of cyclooxygenase 1 and 2, which regulate prostaglandin production. When taken orally, aceclofenac is well absorbed. The maximum concentration is reached in 1.25-3 hours. Aceclofenac enters the joint fluid, where its content reaches 57% of plasma concentrations, and the maximum concentration is reached for 2 to 4 hours longer than in plasma. The volume of distribution of aceclofenac is 25 liters. Aceclofenac binds to plasma proteins (albumins) by 99%. Aceclofenac circulates mainly in unchanged form, its main metabolite is 4′ – hydroxyaceclofenac. the elimination half-life is 4 hours. Aceclofenac is excreted by the kidneys, more than 60% in the form of hydroxy derivatives.
Indications
The drug is prescribed:
- as part of the complex treatment of inflammatory diseases of the musculoskeletal system (osteoarthritis, rheumatoid, psoriatic and juvenile arthritis, podargy, ankylosing spondylitis or spondylitis);
- for the relief of pain and inflammation in rheumatism, toothache, scapular periarthritis.
The use of Aceclofenac is not able to completely cure these diseases, but it quickly reduces inflammation and pain.
Contraindications
The medicine is contraindicated:
- if you are allergic to the Active ingredient;
- patients with ulcers and erosions in the gastrointestinal tract, during an exacerbation;
- for Crohn’s disease and ulcerative colitis;
- if there is suspected bleeding from the gastrointestinal tract;
- in severe liver disease and liver failure;
- pregnant women;
- patients with disorders of hematopoiesis or bleeding disorders;
- during lactation;
- renal failure, hyperkalemia, progressive kidney disease;
- children up to 18 years;
- the period of recovery after conducting coronary artery bypass grafting;
- when “aspirinas triad”;
- persons suffering from severe heart failure.
Caution should be observed:
- in diseases of the kidneys, gastrointestinal tract and liver moderate and mild severity;
- if the patient has the bacterium H. pylori in the gastrointestinal tract;
- bronchial asthma;
- persons with low blood pressure;
- after conducting extensive operations if a reduced blood volume;
- ischemia;
- the elderly;
- patients with cerebrovascular diseases;
- diabetes;
- after prolonged treatment with NSAIDs;
- alcoholics and smokers;
- in severe somatic diseases;
- patients with defects of haemostasis;
- systemic lupus erythematosus;
- in combination with anticoagulants, inhibitors of serotonin reuptake, with antiplatelet agents.
Ointment or cream should not be applied to damaged skin.
Side effects
During treatment with Aceclofenac, the following symptoms are often observed: :
- dizziness, nausea, abnormalities in the gastrointestinal tract, abdominal pain, indigestion, diarrhea;
- increased activity of liver enzymes, allergic reactions in the form of skin rashes and itching.
They develop less frequently:
- anemia, anaphylactic reactions, decreased visual acuity;
- increased blood pressure, heart failure, shortness of breath;
- increased creatinine and urea concentrations in the blood;
- constipation, oral ulcers, increased gas formation, gastritis, vomiting;
- stomach ulcers, melena, hemorrhoids;
- dermatitis, pruritus, angioedema, urticaria.
Very rarely occur:
- muscle spasms, weakness and fatigue, increase in body weight, hyperkalemia, eczema,
- kidney failure, nephrotic syndrome, nephritis;
- purpura, Stevens-Johnson syndrome, soft tissue infections on the background of varicella, toxic epidermal necrolysis;
- granulocytopenia, neutropenia, a violation of hematopoiesis, thrombocytopenia, hemolytic anemia;
- depression, vivid dreams and nightmares, sleep disorders, vertigo, vasculitis;
- tremor, perversion of taste, drowsiness, paresthesia, tinnitus;
- intestinal perforation, pancreatitis, ulcerative colitis, vomiting blood;
- redness of the skin, flushing, tachycardia,
- bronchospasm, stomatitis, hepatitis.
Interaction
Anticoagulants in combination with tablets increase the risk of bleeding. During treatment, it is recommended to periodically monitor blood clotting parameters.
Aceclofenac tablets increase the plasma concentration of digoxin, phenytoin, and lithium-based drugs.
The substance, when combined with antihypertensive agents and diuretics, reduces the effectiveness of the latter.
The combination of the drug with potassium-sparing diuretics increases the likelihood of hyperkalemia.
With caution, the drug should be combined with NSAIDs and glucocorticosteroids, the drug increases the toxicity of methotrexate and cyclosporine.
Aspirin reduces the plasma concentration of Aceclofenac.
The combination of the drug with hypoglycemic drugs requires constant monitoring of the level of glucose in the blood.
How to take, course of use and dosage
Tablets are prescribed inside. Depending on the disease and the patient’s age,100 to 200 mg of the drug is prescribed per day. 100 mg at a time, whole, without dividing or chewing, washed down with plenty of liquid.
Externally. The substance in the ointment or cream is carefully applied in a thin layer to the area of inflammation 3 times a day. After using the drug, it is recommended to wash your hands thoroughly with soap and avoid contact with the eyes.
Overdose
No cases of drug overdose in humans have been reported.
Symptoms of overdose include: headaches, cramps, vomiting, abdominal pain, nausea, and dizziness. Enterosorbents are used as therapy, the stomach is washed, and symptomatic treatment is performed. The drug does not have a specific antidote, hemodialysis and forced diuresis are ineffective.
Special instructions
As a precautionary measure during treatment with the drug, it is recommended to use the minimum effective and active dosages. It also shows periodic monitoring of the liver and kidneys, the picture of peripheral blood, the state of feces (lack of blood), the work of the cardiovascular system.
Do not drive a car or perform potentially dangerous activities during treatment.
The risk of allergies or skin reactions is higher in the first months of taking the medication. If there is damage to the oral mucosa, skin rashes, or other signs of allergies, then therapy should be interrupted.
The process of platelet suppression during treatment with the drug is reversible, the blood composition normalizes some time after the drug is discontinued.
