>of 1 g of cream contains: Active ingredient: silver sulfatiazole-20 mg, Excipients: cetostearyl alcohol (cetyl alcohol 60%, stearyl alcohol 40%), liquid paraffin, white vaseline, glycerol, sodium lauryl sulfate, methyl parahydroxybenzoate, propyl parahydroxybenzoate, potassium dihydrogen phosphate, sodium hydrophosphate, water for injection.
Pharmacological action
Argosulfan cream is a topical antibacterial drug that promotes wound healing (burn, trophic, purulent, etc. ), provides effective protection of wounds from infection, relieves pain and burning in the wound, reduces the time of treatment and preparation of the wound for skin transplantation, and in many cases leads to an improvement in the condition that eliminates the need for transplantation. Sulfanilamide — sulfatiazole, which is part of the cream, has a wide spectrum of antibacterial bacteriostatic action against gram-positive and gram-negative bacteria. The mechanism of antimicrobial action of sulfatiazole – inhibition of microbial growth and reproduction-is associated with competitive antagonism with PABA and inhibition of dihydropteroate synthetase, which leads to disruption of the synthesis of dihydrofolic acid and, ultimately, its active metabolite — tetrahydrofolic acid, necessary for the synthesis of purines and pyrimidines in the microbial cell. The silver ions present in the preparation enhance the antibacterial effect of sulfonamide by several tens of times — they inhibit the growth and division of bacteria by binding to the DNA of the microbial cell. In addition, silver ions weaken the sensitizing properties of sulfonamide. Thanks to the hydrophilic base of the cream, which has an optimal pH and contains a large amount of water, it provides an analgesic effect and hydration of the wound, which contributes to good tolerance, relief and acceleration of wound healing.
Indications
Burns of various degrees, of any nature (thermal, solar, chemical, electric shock, radiation, etc. ), frostbite. Bedsores, trophic ulcers of the lower leg of various origins (including chronic venous insufficiency, obliterating endarteritis, blood supply disorders in diabetes mellitus, erysipelas, etc. ). Purulent wounds, minor domestic injuries (cuts, abrasions). Infected dermatitis, simple contact dermatitis, complicated impetigo, microbial eczema, streptostaphyloderma.
Use during pregnancy and lactation
It is not recommended to use during pregnancy. During breast-feeding, the drug can be prescribed only if the burn surface does not exceed 20% of the body surface, and the therapeutic benefit for the mother is greater than the possible risk for the child.
Contraindications
Hypersensitivity (including to other sulfonamides). Congenital deficiency of glucose-6-phosphate dehydrogenase. Early childhood (newborns and children under 2 months, premature infants) due to the risk of developing nuclear jaundice. It is not recommended to use during pregnancy. During breast-feeding, the drug can be prescribed only if the burn surface does not exceed 20% of the body surface, and the therapeutic benefit for the mother is greater than the possible risk for the child.
Side effects
Skin-allergic reactions. Sometimes irritation is possible, manifested by a burning sensation at the site of application of the cream. With prolonged use, changes in the blood may develop that are characteristic of systemic sulfonamides (for example, leukopenia), desquamative dermatitis.
Interaction
It is not recommended to use together with other local medicines. Folic acid and its structural analogs (for example, procaine) can weaken the antimicrobial effect of sulfatiazole.
How to take, course of use and dosage
Externally, both in an open way and in the form of occlusive dressings. After cleaning and surgical treatment, the drug with a thickness of 2-3 mm is applied to the wound with sterility conditions 2-3 times a day. During treatment, the entire wound should be covered with cream. If part of the wound opens, it is necessary to apply additional cream. Applying an occlusive dressing is possible, but not mandatory. The cream is applied until the wound is completely healed or until the skin is transplanted. If the drug is applied to infected wounds, exudate may appear. Before applying the cream, it is necessary to wash the wound with an antiseptic. The maximum daily dose is 25 g. The maximum duration of treatment is 60 days.
Overdose
No overdose cases have been reported.
Special instructions
With long-term treatment and extensive wound surfaces, the level of sulfatiazole in blood plasma should be monitored, especially in patients with liver and kidney diseases. Caution should be exercised when using in shock patients with extensive burns due to the inability to collect a complete allergic history. Argosulfan cream applied topically does not cause darkening of the skin and underwear. It does not limit the psychophysical ability, the ability to drive vehicles and maintain machinery in motion.
