Pharmacological action
Bicillin-5 is a combined antimicrobial agent with a narrow spectrum of antibacterial action, which is a representative of the penicillin group.
The group is known for its natural origin, as it is produced by some species of molds from the genus Penicillinum. Benzylpenicillin is one of the most persistent representatives of natural penicillins.
Benzylpenicillin has an antibacterial effect against:
- gram-positive microorganisms such as staphylococci, streptococci, enterococci, corynebacteria diphtheria, listeria, clostridium, anaerobic bacilli of the genus Bacilli that form spores;
- gram-negative bacteria: gonococci, meningococci, actinomycetes, spirochetes.
The antibiotic has no antibacterial activity and is not used to treat diseases caused by:
- viruses;Â
- Mycobacterium tuberculosis;
- protozoa;
- rickettsia;
- fungi;
- other Gram-negative microorganisms;
- penicillinase-producing strains.
The mechanism of action is based on the inhibition of a specific enzyme that takes part in the formation of the main structural component of the cell wall. As a result, the strength of the cell wall is disrupted, and pathogenic bacteria are destroyed.
The disadvantages of benzylpenicillin are:
- instability to penicillinases;
- low absorption in the stomach;
- low efficacy against many gram-negative pathogens.
In medicine, for the treatment of infections caused by penicillin-sensitive microorganisms, highly soluble benzylpenicillin salts are used: potassium and sodium. However, due to the short period of their action and rapid elimination, prolonged forms in the form of poorly soluble salts are also used. These are benzathine benzylpenicillin and benzylpenicillin novocaine salt.
Bicillin-5 is a combination of 2 poorly soluble salts. Due to poor dissolution at the injection site, a depot is created, which reduces the frequency of injections, and maintains the concentration of the antibiotic in the blood at the desired level.
The main condition for prescribing Bicillins is the need to maintain effective concentrations of the antibiotic in the blood for a long period of time. They are prescribed for syphilis and other infections caused by treponema, streptococcal infections, such as inflammation of the tonsils, scarlet fever, infected wounds, erysipelas, rheumatism, leishmaniasis.
Indications
Indications for the use of Bicillin-5 are infections caused by bacteria that are sensitive to penicillin, especially when long-term treatment is prescribed:
- scarlet fever;
- erysipelas;
- syphilis of primary, secondary, unknown origin;
- yaws (tropical syphilis);
- rheumatism, rheumatic arthritis, rheumocarditis, rheumatoid diseases;
- tonsillitis, lacunar sore throat, catarrhal sore throat;
- wound infection: infected wounds, burns, purulent wounds, wound botulism.
Recommendations for use
Adults and children from 8 years of age are given the drug 1 time in 4 weeks, children under 8 years of age-1 time in 3 weeks:
- for adults-1.5 million units;
- for children over 8 years of age-1.2-1.5 million units;
- for children under 8 years of age – 600 thousand units.
The contents are diluted under aseptic conditions right before use. To obtain the solution, use water for injection, sodium chloride solution or novocaine. The resulting solution is shaken intensively until a uniform structure is formed and administered intramuscularly.
Contraindications
- hypersensitivity to the active components of the drug or excipients, as well as other penicillins;
- hypersensitivity to other drugs in the anamnesis.
People suffering from bronchial asthma should be prescribed medication with caution.
Side effects
- : anemia, decreased platelet or white blood cell count, decreased blood clotting;
- skin rashes, rashes on the mucous membranes, increased amounts of eosinophils in the blood, dermatitis, angioedema, anaphylaxis;
- headache, joint pain, fever;
- inflammation of the tongue or oral mucosa;
- With prolonged treatment, superinfection or fungal infection may develop.
Special instructions
- the drug should not be administered intravenously;
- the presence of blood in the syringe when injecting evidence enters the vessel, it was necessary to change the injection site;

- when allergic symptoms stop drug use;
- with the development of anaphylaxis take immediate measures administered norepinephrine, glucocorticosteroid drugs artificially ventilate the lungs;
- it is necessary to conduct microbiological, serological tests before starting treatment and for 4 months after a suspected syphilis;
- to prevent the development of fungal infection, we recommend taking the vitamin C and b, sometimes shown nystatin or levorin;
- the use of inadequate doses or discontinuation of therapy before the recommended course leads to the formation of resistant strains of microorganisms,
- antibiotics, bactericidal effect, enhance the effect of Bitsillin-5, and has a bacteriostatic effect – reduce;
- when using the drug reduces the effectiveness of the preformed contraceptive drugs;
- diuretics, allopurinol, phenylbutazone, NSAIDs decrease the excretion of the antibiotic, thus increasing its concentration in the blood.
