Pharmacological action
Pharmacological action-regulating redox processes, increasing the body’s resistance, analgesic, antipyretic, anti-inflammatory.
Pharmacodynamics
Acetylsalicylic acid has analgesic, antipyretic, anti-inflammatory effects associated with the suppression of COX-1 and -2, which regulate the synthesis of PTH; inhibits platelet aggregation.
Ascorbic acid plays an important role in the regulation of redox processes, carbohydrate metabolism, blood clotting, tissue regeneration, and increases the body’s resistance.
Indications
-Symptomatic treatment of moderate or mild pain syndrome of various origins (headache, toothache, migraine, neuralgia, muscle pain, pain during menstruation);
– fever syndrome in colds and other infectious and inflammatory diseases.
Use during pregnancy and lactation
It is contraindicated in the first and third trimester and during breastfeeding.
In the second trimester of pregnancy, it can be prescribed sporadically and with caution.
Contraindications
children’s age (under 15 years), hypersensitivity to any components of the drug and other NSAIDs; erosive-ulcerative lesions of the gastrointestinal tract (in acute phase), gastrointestinal bleeding, inflammatory bowel disease in the acute stage; complete or incomplete combination of bronchial asthma, recurrent polyposis of the nasal mucosa and paranasal sinuses and intolerance of acetylsalicylic acid or other NSAIDs, including in the anamnesis; the combined use of methotrexate in a dose of 15 mg per week or more; pronounced violations of the liver, kidney; expressed by the human heart functions; hemorrhagic diathesis; hemophilia, thrombocytopenia; deficiency of glucose-6-phosphate dehydrogenase; a vitamin K deficiency; cerebrovascular or other bleeding; concomitant use of oral anticoagulants and aspirin in doses exceeding 3 g / day; pregnancy (I and III trimester), lactation.
With caution: with concomitant therapy with anticoagulants, gout, peptic ulcer of the stomach and/or duodenum (in the anamnesis), erosive gastritis, and a tendency to gastrointestinal bleeding, liver function disorders, hypotrombinemia, hypovitaminosis K, anemia, conditions predisposing to fluid retention in the body (including cardiac dysfunction, arterial hypertension), thyrotoxicosis, propensities to calcium-oxalate nephrolithiasis. Bronchial asthma, nasal polyposis, pollinosis, drug allergy, hypermenorrhea.
Side effects
Gastrointestinal disorders: often-decreased appetite; rarely-diarrhea; frequency unknown-dyspepsia, abdominal pain, nausea, vomiting, obvious (black stools, bloody vomiting) or hidden signs of gastrointestinal bleeding, which can lead to iron deficiency anemia, erosive and ulcerative lesions (including perforation) of the gastrointestinal tract. Nervous system disorders: infrequently-headache; frequency unknown-dizziness, tinnitus (usually signs of overdose). Blood and lymphatic system disorders: frequency unknown-hemorrhagic syndrome, thrombocytopenia. Bleeding can lead to acute or chronic anemia, iron deficiency anemia with appropriate laboratory and clinical manifestations (asthenia, pallor, hypoperfusion). Patients with glucose-6-phosphate dehydrogenase deficiency may experience hemolysis and hemolytic anemia. Renal and urinary tract disorders: frequency unknown – when used in high doses-hyperoxaluria and formation of urinary stones from calcium oxalate, damage to the glomerular apparatus of the kidneys. Immune system disorders: frequency unknown-skin rash, rhinitis, anaphylactic reactions, bronchospasm, angioedema, formation of the “aspirin triad” (bronchial asthma, polypous rhinosinusitis and intolerance to acetylsalicylic acid and pyrazolone preparations). Liver and biliary tract disorders: very rarely – Reye’s syndrome (acute fatty liver with advanced acute liver failure and encephalopathy); rarely-liver function disorders (increased hepatic transaminases). If such symptoms occur, it is recommended to stop taking the drug and immediately consult your doctor.
Interaction
Increases the toxicity of methotrexate, the effects of narcotic analgesics, other NSAIDs, oral hypoglycemic drugs, heparin, indirect anticoagulants, thrombolytics and platelet aggregation inhibitors, sulfonamides (including co-trimoxazole), triiodothyronine, reserpine; reduces the effects of uricosuric drugs (benzbromarone, sulfinpyrazone), antihypertensive agents and diuretics (spironolactone, furosemide).
Increases the concentration of digoxin, barbiturates and lithium preparations in the blood plasma.
CORTICOSTEROIDS, alcohol and alcohol-containing drugs increase the damaging effect on the gastrointestinal mucosa, increase the risk of gastrointestinal bleeding.
Antacids containing magnesium and / or aluminum hydroxide slow and impair the absorption of acetylsalicylic acid.
Ascorbic acid improves the absorption of iron preparations in the intestine.
How to take, course of use and dosage
Inside. Dissolve the effervescent tablet in a glass of water and drink it after a meal. For mild to moderate pain syndrome and febrile conditions, a single dose is 1-2 effervescent tablets, the maximum single dose is 2 effervescent tablets, the maximum daily dose should not exceed 6 tablets. Intervals between doses of the drug should be at least 4 hours. The duration of treatment (without consulting a doctor) should not exceed 3-5 days. In order to reduce the risk of adverse reactions from the gastrointestinal tract, the minimum effective dose of the drug should be used in the shortest course of treatment.
Overdose
Symptoms: Â with mild intoxication, nausea, vomiting, epigastric pain, as well as (especially in children and elderly patients) tinnitus, dizziness, headaches, decreased vision and hearing are possible.
With a significant overdose, incoherent thinking, confusion, drowsiness, collapse, tremor, shortness of breath, suffocation, dehydration, hyperthermia, coma, alkaline urine reaction, metabolic acidosis, respiratory (gas) alkalosis, and carbohydrate metabolism disorders appear.
The lethal dose of acetylsalicylic acid for adults is more than 10 g, for children – more than 3 g.
Treatment: Â depending on the state of acid-base balance and electrolyte balance, solutions of sodium bicarbonate, sodium citrate or sodium lactate are infused.
Special instructions
The drug is not prescribed as an antipyretic for children under 15 years of age with acute respiratory diseases caused by viral infections, because of the risk of developing Reye’s syndrome.
Acetylsalicylic acid reduces the excretion of uric acid from the body, which can cause an acute attack of gout in predisposed patients.
Possible drug interactions.
Glucocorticosteroids, alcohol increase the damaging effect on the gastrointestinal mucosa. One dose of the drug contains 933 mg of sodium, which should be considered in patients receiving a salt-free diet.
In order to reduce the risk of increased bleeding, the drug should be discontinued 4-8 days before elective surgery.
