Pharmacological action
Anti-allergic drug, selective blocker of peripheral histamine H1-receptors. Loratadine is a tricyclic compound with a pronounced antihistamine effect.
It has a fast and long-lasting anti-allergic effect. Loratadine does not penetrate the BBB and does not affect the central nervous system. It does not have a clinically significant anticholinergic or sedative effect, i. e. it does not cause drowsiness and does not affect the rate of psychomotor reactions when used at the recommended doses.
Taking Claritine does not lead to prolongation of the QT interval on the ECG. During long-term treatment, no clinically significant changes in vital signs, physical examination data, laboratory results, or ECG were observed. Loratadine has no significant selectivity for histamine H2 receptors.
It does not inhibit norepinephrine reuptake and has virtually no effect on the cardiovascular system or pacemaker function.
After oral use of Claritine, the onset of action is within 30 minutes. The antihistamine effect reaches its maximum after 8-12 hours from the start of action and lasts for more than 24 hours.
Indications
Allergic rhinitis, conjunctivitis and elimination of symptoms associated with these diseases – sneezing, itching of the nasal mucosa, rhinorrhea, burning and itchy eyes, lacrimation; seasonal pollinosis; chronic idiopathic urticaria;allergic skin diseases.
Use during pregnancy and lactation
The safety of using loratadine during pregnancy has not been established.
The use of Claritine® during pregnancy is possible only if the intended benefit to the mother exceeds the potential risk to the fetus.
Loratadine and its active metabolite are excreted in breast milk, so when prescribing the drug during lactation, you should decide whether to stop breastfeeding.
Contraindications
– Hypersensitivity to any component of the drug, the age of 2 years (for the syrup), up to 3 years or weighing less than 30 kg (for tablets);
– the period of breastfeeding;
– a rare hereditary disease (impaired tolerance to galactose, lactase deficiency) for tablets; glucose-galactose malabsorption;
– deficiency of sucrase/isomaltase, intolerance of fructose syrup.
With caution: severe hepatic impairment.
Side effects
Headache (2.7%), nervousness (2.3%), and fatigue (1%) were observed more frequently in clinical trials involving children aged 2 to 12 years taking Claritine® than in the placebo group.
Nervous system disorders: in children aged 2-12 years – headache (2.7%), nervousness (2.3%), fatigue (1%); in adults – headache (0.6%), drowsiness (1.2%), insomnia (0.1%).

Interaction
Claritine does not enhance the effect of ethanol (alcohol) on the central nervous system.
When Claritine was co-administered with ketoconazole, erythromycin or cimetidine, an increase in the concentration of loratadine and its metabolite in plasma was noted, but this increase was not manifested clinically, including according to ECG data.
How to take, course of use and dosage
Apply regardless of the meal time.
Adults, including the elderly, and adolescents over 12 years of age are recommended at a dose of 10 mg (1 tablet or 2 teaspoons (10 ml) of syrup) 1 time a day.
Children from 2 to 12 years of age are recommended to prescribe the dose of the drug depending on body weight: with a body weight of 30 kg or less – 5 mg (1 teaspoon (5 ml) of syrup) 1 time a day; with a weight of more than 30 kg – 10 mg (2 teaspoons (10 ml) of syrup or 1 tablet) 1 time a day.
For adults and children with severe hepatic impairment, the initial dose should be: with a body weight of 30 kg or less – 5 mg (1 teaspoon (5 ml) of syrup) every other day, with a body weight of more than 30 kg-10 mg (2 teaspoons (10 ml) of syrup or 1 tablet) every other day.
Overdose
Symptoms: drowsiness, tachycardia, headache.
Treatment: gastric lavage, intake of adsorbents (crushed activated carbon with water), symptomatic and supportive therapy. Loratadine is not eliminated by hemodialysis. After providing emergency care, it is necessary to continue monitoring the patient’s condition.

Special instructions
Claritine® does not have a clinically significant sedative effect when used at the recommended doses (10 mg once a day). The sedative profile of loratadine when taken 10 mg once a day is comparable to placebo. A dose-dependent increase in drowsiness was observed at doses 2 to 4 times higher than the recommended dose of 10 mg. Claritine® does not potentiate the effects of alcohol.
Claritine® should be discontinued 48 hours prior to skin diagnostic allergy tests to prevent false results. Patients with severe hepatic impairment should be given a lower initial dose due to a possible decrease in loratadine clearance (the recommended initial dose is 5 mg once a day or 10 mg every other day).
When Claritine® was administered for 90 days at a dose exceeding the therapeutic dose by 4 times, no clinically significant prolongation of the QT interval was detected on the electrocardiogram. Claritine® contains lactose. Therefore, it should not be prescribed to patients suffering from rare hereditary diseases associated with galactose intolerance, Lapland lactase deficiency or glucose-galactose malabsorption.
At the recommended doses, Claritine® does not affect the ability to drive vehicles or complex technical devices. However, patients should be advised of the potential risk of drowsiness in the event of overdose.
