Pharmacological action
A bacteriostatic antibiotic from the lincosamide group, it has a broad spectrum of action, binds to the 50S subunit of the ribosomal membrane and suppresses protein synthesis in the microbial cell.
In relation to a number of gram-positive cocci, a bactericidal effect is possible.
Active against Staphylococcus spp. (including Staphylococcus epidermidis, and reproducirse producing penicillinase), Streptococcus spp. (except Enterococcus faecalis), Streptococcus pneumoniae, Corynebacterium diphtheriae, Mycoplasma spp., anaerobic and microaerophilic gram-positive cocci (including Peptococcus spp. and Peptostreptococcus spp. ), Clostridium perfringens, Clostridium tetani, Bacteroides spp. (including Bacteroides fragilis and Prevotella melaninogenica), Fusobacterium spp., Propionibacterium spp. Eubacterium spp. and Actinomyces israelii.
Most strains of Clostridium perfringens are sensitive to clindamycin, but others are not. species of Clostridium spp. (including Clostridium sporogenes, Clostridium tertium) are resistant to its action, and therefore, for infections caused by Clostridium spp., it is recommended to determine the antibioticogram. There is cross-resistance between clindamycin and lincomycin.
Indications
Vaginitis caused by drug-sensitive microorganisms.
Use during pregnancy and lactation
During pregnancy, it is possible if the expected effect of therapy exceeds the potential risk to the fetus (adequate and strictly controlled studies in pregnant women have not been conducted, clindamycin passes through the placenta and can be concentrated in the fetal liver, but no complications have been reported in humans).
Studies have not established whether treatment for bacterial vaginosis reduces the risk of adverse pregnancy outcomes such as premature rupture of the membranes, premature onset of labor, or premature delivery.
Caution should be exercised when using during breastfeeding (it is not known whether clindamycin penetrates into breast milk after external and intravaginal use, but it is detected in breast milk after oral or parenteral use).
Contraindications
Hypersensitivity, pregnancy I trimester (for vaginal cream).
Side effects
Allergic reactions (including skin rash, urticaria).
Irritation at the site of application, cervicitis, vaginitis, development of superinfection (fungal vaginitis).
Interaction
Incompatible with solutions containing a complex of B vitamins, aminoglycosides, ampicillin, phenytoin, barbiturates, aminophylline, calcium gluconate and magnesium sulfate.
In vitro antagonism of clindamycin with erythromycin, as well as with chloramphenicol, is shown; since this effect may be clinically significant, these drugs should not be taken simultaneously.
Since clindamycin interferes with neuromuscular transmission and may enhance the effect of peripheral muscle relaxants, caution should be exercised when used concomitantly and the patient should be closely monitored.
When used concomitantly with opioid (narcotic) analgesics, respiratory depression may increase, up to apnea. Concomitant use with antidiarrheal drugs that reduce gastrointestinal motility increases the risk of pseudomembranous colitis.
How to take, course of use and dosage
Intravaginally: vaginal cream is prescribed in a single dose – 100 mg of clindamycin (1 applicator) at night, the course of treatment is 3-7 days.
Overdose
Symptoms: Â increased severity of side effects.
Treatment: Â symptomatic and supportive treatment. It is not excreted by hemodialysis or peritoneal dialysis.
Special instructions
Vaginal cream is not recommended to be used simultaneously with other intravaginal drugs.
