Pharmacological action
Diclofenac is a nonsteroidal anti-inflammatory drug (NSAID). It has anti-inflammatory, analgesic and antipyretic effects. Inhibits the COX enzyme in the arachidonic acid metabolism cascade and disrupts prostaglandin biosynthesis.
When applied externally, it has an anti-inflammatory and analgesic effect.
Reduces and relieves pain at the site of application of the ointment (including joint pain at rest and when moving), reduces morning stiffness and swelling of the joints. Helps to increase the volume of movement.
Indications
- inflammatory and degenerative joint diseases (osteochondrosis, osteoarthritis, periarthropathies);
- soft tissue rheumatism;
- traumatic bruises;
- sprains of ligaments, muscles and tendons;
- inflammatory edema of soft tissues;
- arthralgia and myalgia caused by heavy physical exertion.
Contraindications
- pathological changes in the blood picture of unknown origin;
- ulcer disease of stomach and duodenum;
- destructive and inflammatory bowel disease in the acute phase ;
- pregnancy;
- lactation (should abandon breastfeeding);
- children’s and teenage age up to 12 years (in/m introduction, supp. );
- children up to age 6 years (tablets, ointment);
- hypersensitivity to acetylsalicylic acid or other NSAIDs (including “aspirin triad”);
- hypersensitivity to diclofenac and other components of the drug;
- inflammation of the rectum – practice (suppositories)
Side effects
Skin rash, burning, and redness may occur at the site of application.
With prolonged external use of the drug, systemic side effects from the digestive system, central nervous system, respiratory system, as well as allergic reactions may develop.
Interaction
Concomitant use of Diclofenac with digoxin, phenytoin or lithium preparations may increase the plasma concentrations of these drugs; with diuretics and antihypertensive agents, the effect of these drugs may decrease; with potassium-sparing diuretics, hyperkalemia may develop; with acetysalicylic acid, a decrease in the concentration of diclofenac in blood plasma and an increased risk of side effects.
Diclofenac may increase the toxic effect of cyclosporine on the kidneys.
Diclofenac can cause hypo – or hyperglycemia, so when used concomitantly with hypoglycemic agents, monitoring of blood glucose concentration is required.
When using methotrexate for 24 hours before or after taking Diclofenac, it is possible to increase the concentration of methotrexate and increase its toxic effect.
When used concomitantly with anticoagulants, regular monitoring of blood clotting parameters is necessary.
How to take, course of use and dosage
In a dose of 2-4 g, apply a thin layer on the skin above the focus of inflammation and lightly rub, the frequency of use is 2-3 times a day.
The maximum daily dose of ointment should not exceed 8 g.
The course of treatment is no more than 14 days. The need for longer use of the drug is determined by the doctor.
Special instructions
Avoid contact with Diclofenac ointment on the damaged skin surface, open wounds, eyes and mucous membranes.
The use of Diclofenac in the form of an ointment is possible only as prescribed by a doctor in the following cases: a history of peptic ulcer of the stomach and duodenum; liver and kidney function disorders; severe hematopoietic disorders; bronchial asthma, rhinitis, nasal mucosal polyps; a history of allergic reactions.
