Pharmacological action
Erythromycin is an antibacterial substance from the macrolide group that is produced by Streptomyces erythreus. The effect of the drug is bacteriostatic.
It has an antibacterial effect on a large number of gram-positive and Gram-negative microorganisms (various species of N. gonorrhoeae, N. Meningitidis, Streptococcus, Staphylococcus), including strains of bacteria resistant to other antibiotics. Erythromycin is also active against Actinomyces israelii, Mycoplasma pneumoniae, Ciostridium spp., Corynebacterium diphtheriae, Listeria monocytogenes, Haemophilus influenzae, and chlamydia.
Most gram-negative bacteria, viruses, mycobacteria, and fungi are resistant to the drug.
The tolerance of Erythromycin is better than that of penicillins, so it can be prescribed for allergy to the latter. Resistance to erythromycin microflora develops rapidly.
Indications
- conjunctivitis (including in newborns),
- neonatal ophthalmia,
- bacterial blepharitis,
- blepharoconjunctivitis,
- keratitis,
- meibomitis (barley),
- chlamydia,
- trachoma.
Contraindications
- severe hepatic impairment;
- hypersensitivity to the components of the drug.
Side effects
When applying the ointment, an irritating effect (redness, itching) is possible.
With prolonged use, a secondary infection caused by erythromycin-resistant microorganisms may develop.
Interaction
Erythromycin is an antagonist of lincomycin, clindamycin, and chloramphenicol. It can significantly reduce the bactericidal effect of beta-lactam antibacterial agents (penicillins, cephalosporins, carbopenems).
Simultaneous use of medicinal external forms of erythromycin and abrasive substances that cause skin irritation, or with products that cause skin exfoliation, may manifest a cumulative irritating or drying effect.
How to take, course of use and dosage
Ointment should be placed behind the lower eyelid three times a day, in the treatment of trachoma up to 5 times a day. The duration of the course of therapy depends on the severity and form of the disease, but it should not exceed 2 weeks.
Treatment of trachoma with erythromycin should be combined with the opening of purulent follicles. When the inflammation process subsides, the drug is used three times a day. The course of treatment for trachoma should not exceed three months.

Special instructions
With caution, the drug is prescribed for impaired liver and/or kidney function.
Erythromycin can be prescribed for penicillin allergy patients. Erythromycin is not recommended to be washed down with milk and dairy products. Erythromycin passes through the placenta and is excreted in breast milk, so when prescribing the drug during pregnancy and lactation, the intended benefit to the mother and the potential risk to the fetus should be evaluated.
To prevent neonatal ophthalmia, it is not recommended to wash out eye ointment from the eye. In children whose mothers have severe gonorrhea, erythromycin is recommended to be used simultaneously with a solution of penicillin G (water) for parenteral use.
