Pharmacological action
Ferretab comp. – a tool for the treatment and prevention of iron deficiency conditions.
The action is determined by the properties of the active substances included in its composition.
Iron fumarate is a salt of iron, a trace element necessary for the synthesis of hemoglobin. When using iron in the form of salts, its deficiency in the body is quickly replenished, which leads to a gradual regression of clinical (weakness, fatigue, dizziness, tachycardia, soreness and dry skin) and laboratory symptoms of anemia.
Folic acid is essential for the normal maturation of megaloblasts and the formation of normoblasts. It stimulates erythropoiesis, participates in the synthesis of amino acids, nucleic acids, purines and pyrimidines, and in choline metabolism. During pregnancy, it protects the body from the effects of teratogenic factors.
The drug helps maintain and restore normal iron levels in the blood, and also prevents complications such as anemia, miscarriage, early birth and impaired mental development of the child, which can be observed with a lack of iron and folic acid in the body. The amount of iron absorbed varies between 5 and 35%.
In serum, iron binds to transferrin, is involved in the formation of hemoglobin, myoglobin, cytochrome oxidase, catalase and peroxidase, or is stored in body tissues as ferritin.
Pharmacokinetics
The release of iron occurs in the feces, urine, and sweat.
Folic acid is absorbed mainly in the upper intestine (duodenum) and binds to proteins by 64%. It is metabolized in the liver and excreted mainly through the kidneys, but also partially in the feces.
Indications
Treatment and prevention of iron deficiency conditions (pregnancy, malabsorption of iron in the gastrointestinal tract, prolonged bleeding, unbalanced and inadequate nutrition).
Use during pregnancy and lactation
Application is possible if there are indications.
Contraindications
- Hypersensitivity to the components of Ferretab comp. ;
- diseases associated with the accumulation of iron in the body (aplastic and hemolytic anemia, thalassemia, hemosiderosis, hemochromatosis);
- impaired iron absorption (sideroachrestic anemia, lead anemia, pernicious anemia (vitamin B12 deficiency);
- anemia not caused by a lack of iron or folic acid.
Side effects
It is usually well tolerated.
In some cases, there may be a temporary adverse effect on the gastrointestinal tract — a feeling of fullness and unpleasant sensations in the stomach, nausea, vomiting, allergic reactions, constipation.
Interaction
When taken concomitantly with antacids, iron absorption worsens.
Concomitant use with tetracycline antibiotics should be avoided due to iron malabsorption.
Cholestyramine, phenytoin, phenobarbital, carbamazepine, sulfasalazine, hormonal contraceptives, folic acid antagonists, trimethoprim, triamterene, solid foods, bread, raw cereals, dairy products, eggs, tea-reduce iron absorption.
How to take, course of use and dosage
of Ferretab comp. taken orally, on an empty stomach, abundantly washed down with liquid,1 capsule per day.
If there is a pronounced lack of iron or folic acid, the dose should be increased to 2-3 capsules per day.
Maintenance treatment should be continued for at least 4 weeks after reaching normal hemoglobin levels to normalize serum ferritin, which reflects the body’s iron stores.
Overdose
Symptoms: increased manifestations of side effects.
Treatment: conducting a study of serum iron content and serum ferritin level.
In case of significant excess of normal serum ferritin values, an iron chelator (desferal) should be prescribed in accordance with the manufacturer’s recommended dose.
Special instructions
Before starting iron therapy, it is necessary to conduct a study of the level of serum iron, serum ferritin (i. e., document iron deficiency).
When taking the drug, dark fecal staining is possible, and the benzidine test is false positive.
