Pharmacological action
Flucinar is a synthetic corticosteroid for external use. It has anti-inflammatory, antipruritic, anti-allergic, and antiexudative effects.
The mechanism of anti-inflammatory action is not fully understood. Fluocinolone acetonide inhibits the synthesis of prostaglandins and leukotrienes by inhibiting the activity of phospholipase A2 and reduces the release of arachidonic acid from the phospholipids of the cell wall. Prevents the appearance of local allergic reactions in the skin. The anti-exudative effect is due to the vasoconstrictor effect.
Pharmacokinetics
Easily penetrates the skin through the stratum corneum. It does not undergo biotransformation in the skin and accumulates in the stratum corneum. Fluocyolone acetonide is detected in the skin even after 15 days from the moment of its topical application.
When applied externally, fluocinolone acetonide may be slightly absorbed into the systemic circulation and cause general phenomena. The absorbed part of biotransforms mainly in the liver and is released from the body in the form of compounds with glucuronic acid and in a small amount in an unbound form. It is excreted mainly in the urine and slightly in the bile.
Absorption of fluocinolone acetonide is more pronounced when applied to the face and affected areas of the skin, as well as when using occlusive dressings. In addition, the absorption of the drug increases with frequent use and when applied to large areas of the skin. The absorption of the drug in children is more pronounced than in adults.
Indications
Acute and severe uninfected, dry inflammatory skin diseases of various origins, accompanied by pruritus and hyperkeratosis, treatable with corticosteroids:
- seborrheic dermatitis;
- contact eczema;
- atopic dermatitis;
- psoriasis (including psoriasis of the scalp);
- erythematous lichen;
- erythema multiforme;
- lichen planus;
- impetiginous eczema.
Contraindications
- Hypersensitivity;
- acne;
- bacterial (including tuberculosis), fungal, viral and tumor skin diseases.
Side effects
Atrophy of the skin and subcutaneous fat, lupus-like syndrome with a mild course, the appearance or exacerbation of symptoms of lichen planus, epithelialization disorders, slowing healing of wounds and ulcerative lesions, petechiae, steroid purpura, striae, hirsutism, baldness in women, maculopapular skin rash.
How to take, course of use and dosage
Externally. Apply to the affected surface, lightly rubbing,1-2 times a day, no more than 2 g / day. For psoriasis, an occlusive dressing may be used. Duration of the course-no more than 1-2 weeks. On the skin of the face, apply very carefully and no more than 1 week.
Children older than 2 years — with caution, only once a day, on small areas of the skin.
Overdose
Symptoms: Â if the dosage regimen is not followed (prolonged use of the drug on large areas of the skin), systemic side effects of the drug may develop, such as increased blood pressure, edema, and immunosuppressive effects.
Treatment: Â discontinuation of the drug.
Special instructions
Long-term use of Flucinar should be avoided. With prolonged use of corticosteroids on large areas of the skin, the frequency of adverse reactions in the form of peripheral edema, arterial hypertension, hyperglycemia, and immunosuppressive effects increases.
If a secondary infection develops at the site of application of the drug Flucinar, antibacterial and antifungal therapy should be prescribed.
With extreme caution, the drug should be used on the skin of the eyelids or in the eye area in patients with glaucoma and cataracts, due to the possible increase in symptoms of the disease.
When applied to the face, the absorption of the drug increases and even after short-term use, the possibility of side effects (telangiectasia, skin atrophy, perioral dermatitis) increases. Use the drug in this area should be extremely careful.
Caution should be exercised when prescribing Flucinar to people with existing atrophic skin changes, especially elderly patients.
