Pharmacological action

Folacin is a vitamin of group B. In the body, it is converted to tetrahydrofolic acid, which is involved in various metabolic processes as a coenzyme and is necessary for the normal maturation of megaloblasts and the formation of normoblasts. When folic acid deficiency develops megaloblastic type of hematopoiesis. It stimulates erythropoiesis, participates in the synthesis of amino acids (including methionine, serine, glycine), nucleic acids, purines, pyrimidines, and in the exchange of choline and histidine. During pregnancy, it performs a protective function in relation to the action of teratogenic and damaging factors of the fetus. Promotes normal maturation and functioning of the placenta.

Pharmacokinetics

After oral use, folic acid is absorbed into the gastrointestinal tract and almost completely binds to plasma proteins. It is deposited and metabolized in the liver to form tetrahydrofolic acid (in the presence of ascorbic acid under the action of the enzyme dihydrofolate reductase).

Penetrates through the BBB, placenta and breast milk. T_max in the blood is 30-60 minutes. It is excreted by the kidneys both in unchanged form (if the dose taken significantly exceeds the daily requirement) and in the form of metabolites. It is removed by hemodialysis. 5 mg of folic acid taken orally is eliminated from the body after 5 hours.

Indications

• Treatment and prevention of folate deficiency on the background unbalanced or defective power supply;
• treatment and prevention of anemia in the background of folic acid deficiency: macrocytic hyperchromic anemia, anemia, and leukopenia caused by drugs and ionizing radiation, megaloblastic anemia, post-resection anemia, sideroblastic anemia in the elderly, anemia is associated with diseases of the small intestine, sprue and malabsorption syndrome;
• treatment and prevention of anemia during pregnancy and lactation;
• pregnancy — prevention of the development of neural tube defects in the fetus;
• long-term treatment with folic acid antagonists (methotrexate, a combination of sulfamethoxazole and trimethoprim), anticonvulsant drugs (phenytoin, primidon, phenobarbital).

Contraindications

• Hypersensitivity to the components of Folacin;
• pernicious anemia;
• malignant neoplasms;
• cobalamin deficiency.

Side effects

From the digestive system: anorexia, nausea, bloating, bitterness in the mouth.

Allergic reactions: skin rashes, pruritus, erythema, bronchospasm.

Interaction

When used concomitantly with chloramphenicol, neomycin, polymyxins, tetracyclines, folic acid absorption decreases.

When used concomitantly, folic acid reduces the effects of oral contraceptives, ethanol, sulfasalazine, cycloserine and glutetimide, methotrexate, phenytoin, primidone, and chloramphenicol.

Folic acid supplementation may reduce plasma levels of phenobarbital, phenytoin, or primidone and cause an epileptic seizure.

A decrease or change in absorption is possible with the simultaneous use of colestyramine and folic acid, so folic acid should be taken 1 hour before or 4-6 hours after colestyramine.

How to take, course of use and dosage

of Folacin is prescribed orally.

For the treatment of megaloblastic anemia caused by folic acid deficiency: 5 mg per day for 4 months, for prevention-2.5 mg per day.

For the prevention and treatment of macrocytic anemia in malabsorption, inflammatory bowel diseases and unbalanced or inadequate nutrition-15 mg per day, patients with sprue disease-5-15 mg per day.

To prevent the development of neural tube defects in the fetus – 2.5 mg per day for 4 weeks before the expected pregnancy. Reception is continued during the first trimester of pregnancy.

Higher maintenance doses of Folacin may be given to patients with alcoholism, as well as to patients with chronic infections and taking anticonvulsants.

Special instructions

Patients undergoing hemodialysis require increased amounts of folic acid.

With prolonged use of folic acid, especially in high doses, it is possible to reduce the concentration of vitamin B12 in the blood.

Long-term use of the drug is recommended to be combined with taking vitamin B12 (cyanocobalamin).
