Pharmacological action
Chorionic gonadotropin is a gonadotropic hormone. It is produced by the human placenta and excreted in the urine, from where it can be extracted and purified. It stimulates the production of progesterone by the corpus luteum and supports the development of the placenta. It has a gonadotropic effect, mainly luteinizing.
In women, the drug causes ovulation and stimulates the synthesis of estrogens and progesterone. In men, it stimulates spermatogenesis and the production of sex steroids.
Pharmacokinetics
After intravenous use, it is well absorbed. T1 / 2 is 8 hours.
It is believed that before excretion in the urine, chorionic gonadotropin is modified in the body, since its T1 / 2, measured by the immunological method, significantly exceeds this indicator, determined by biological activity.
Indications
For women:
Induction of ovulation after stimulation of follicle growth;
Maintenance of ovarian corpus luteum function in patients with luteal phase insufficiency.
For boys and men: Â
Cryptorchidism (ectopia, retention of testicles in the abdominal cavity or inguinal canal);
Delayed puberty;
Hypogonadotropic hypogonadism (in combination with human menopausal gonadotropin preparations);
When conducting a differential diagnostic test of anorchism and cryptorchidism in boys;
When performing a Leydig functional test to assess testicular function in hypogonadotropic hypogonadism before starting long-term stimulating treatment.
Contraindications
- Tumors of various localization, depending on sex hormones;
- Organically caused cryptorchidism (inguinal hernia, consequences of surgical interventions in the groin area, incorrect position of the testicles) for boys and men.
- Hypersensitivity to chorionic gonadotropin.
Side effects
From the endocrine system in women: ovarian hyperstimulation syndrome may develop, accompanied by the development of ovarian cysts with the risk of their rupture, ascites, hydrothorax and the risk of thromboembolism.
Endocrine disorders in boys and men: Â possible temporary reversible enlargement of the mammary glands, prostate, water and electrolyte retention, acne vulgaris;
Boys may have behavioral changes similar to those observed during the first phase of puberty, which pass after the end of treatment.
How to take, course of use and dosage
Chorionic gonadotropin is prescribed intramuscularly. The dose is individualized depending on the nature, severity of the disease and the age of the patient and ranges from 500 to 3000 units per day.
For men, the drug is administered 2-3 times a week in courses of 4 weeks with intervals of 4-6 weeks. Conduct 3-6 courses within 6-12 months. Long-term continuous use of the drug is not recommended due to the risk of antibody formation and suppression of the gonadotropic function of the pituitary gland.
In anovulatory cycles with a high level of estrogens in women, which indicates normal maturation of follicles, the drug is administered starting from the 10th-12th day of the cycle at 3000 units 2-3 times with an interval of 2-3 days or 1500 units 6-7 times every other day.
In case of interstitial-pituitary insufficiency, treatment with chorionic gonadotropin is best performed after preliminary treatment in the first phase of the cycle with follicle-stimulating hormone. With pituitary nanism with the phenomena of sexual infantilism, the drug is administered at a dose of 500-1000 UNITS 1-2 times a week for 1-2 months in repeated courses.
Gonadotropin treatment for chorionic cryptorchidism in children is most effective at the age of 10 years. The dosage and duration of treatment are individual, depending on the location of the undescended testicle and the nature of cryptorchidism (one – or two-sided).
The most common treatment regimen is as follows: for cryptorchidism in children under the age of 10 years,500-1000 units are administered, and at the age of 10-14 years,1500 units are administered 2 times a week for 4-6 weeks in repeated courses.
For diagnostic purposes, the drug is administered to men at 1500-3000 units per day for 5 days. With a positive test result, the daily urinary excretion of 17-ketosteroids and testosterone increases.
