Pharmacological action
Combined preparation for external use, the action of which is determined by the properties of its constituent components.
Heparin is a direct-acting anticoagulant that belongs to the group of medium-molecular-weight heparins.
When applied externally, it has a local anticoagulant, anti-exudative, moderate anti-inflammatory effect.
Blocks the formation of thrombin. Heparin reduces the inflammatory process and has an anticoagulant effect, indirectly improves microcirculation, which contributes to the process of resorption of hematomas and blood clots and reduces tissue swelling.
Benzyl nicotinate is a peripheral vasodilating agent that improves microcirculation. It is used as an adjuvant to improve the penetration of heparin.
Benzocaine has a local analgesic effect.
Reduces the permeability of the cell membrane for sodium ions, displaces calcium ions from the receptors located on the inner surface of the membrane, blocks the conduction of nerve impulses.
Prevents the occurrence of pain impulses in the endings of sensitive nerves and their conduction through nerve fibers.

Clinical pharmacology
Heparin is slightly absorbed from the surface of the skin. Plasma protein binding — up to 95%, volume of distribution-0.06 l / kg. It does not pass through the placenta and into breast milk. It is intensively absorbed by endothelial cells and cells of the mononuclear-macrophage system, and is concentrated in the liver and spleen. It is metabolized in the liver with the participation of N-desulfamidase and platelet heparinase, which is involved in the metabolism of heparin at later stages. Desulfated molecules are converted into low-molecular-weight fragments under the influence of renal endoglycosidase. Half-life (T 1/2) — 1-6 hours (on average-1.5 hours); increases with obesity, liver and/or kidney failure; decreases with pulmonary embolism, infections, malignant tumors. It is excreted by the kidneys, mainly in the form of inactive metabolites. It is not excreted by hemodialysis. Benzyl nicotinate penetrates all layers of the skin. It practically does not enter the systemic circulation. Benzocaine with external use is practically not absorbed.

Contraindications
Hypersensitivity to the components of the drug; violation of the integrity of the skin of the affected area (open infected wounds, ulcerative-necrotic processes in particular); children under 1 year of age. With caution, disorders of the blood coagulation system, thrombocytopenia.
Side effects
With prolonged use of the drug, local reactions are possible in the form of skin hyperemia, decreased sensitivity at the site of application, allergic reactions (skin rash, itching).
How to take, course of use and dosage
Externally.
The ointment is applied in a thin layer to the affected area (at the rate of 0.5-1 g (2-4 cm of ointment) to an area with a diameter of 3-5 cm) and gently rubbed into the skin.
The ointment is applied 2-3 times a day daily until the inflammatory phenomena disappear, on average from 3 to 7 days. The possibility of a longer course of treatment is determined by the doctor.
With thrombosis of external hemorrhoids, rectal swabs are used, the ointment is applied to a calico or linen pad, which is applied directly to the thrombosed nodes and fixed.
A swab soaked in ointment is inserted into the anus. The ointment should be applied daily, until symptoms disappear, on average 3-14 days.

Special instructions
With prolonged use of Heparin ointment in large areas and simultaneous use of oral medications that affect the blood coagulation system and/or hematopoiesis, the clotting time and prothrombin time should be monitored. If a long course of treatment is necessary and extensive areas of the lesion are present in pregnant women and in women over the age of 65, similar monitoring is recommended.
