Pharmacological action
An antiprotozoal and antimicrobial drug derived from 5-nitroimidazole. The mechanism of action consists in the biochemical reduction of the 5-nitro group of metronidazole by intracellular transport proteins of anaerobic microorganisms and protozoa. The reduced 5-nitro group of metronidazole interacts with the cell DNA of microorganisms, inhibiting the synthesis of their nucleic acids, which leads to the death of bacteria.
Active against Trichomonas vaginalis and gram-negative anaerobes Bacteroides spp. (including Bacteroides fragilis, Bacteroides distasonis, Bacteroides ovatus, Bacteroides thetaiotaomicron, Bacteroides vulgatus), Fusobacterium spp. and some gram-positive microorganisms (sensitive strains of Eubacterium spp., Clostridium spp., Peptococcus spp., Peptostreptococcus spp. ). The MPC for these strains is 0.125-6.25 mcg / ml.
When applied externally, it has an anti-acne effect, the mechanism of which is not exactly known (it is not related to the effect on the Demodex folliculorum mite found in hair follicles and sebaceous gland secretions, and any effect on the production of this secretion).
Metronidazole for external use may have antioxidant activity. It was found that it significantly reduces the production of active oxygen, hydroxyl radicals, and hydrogen peroxide by neutrophils, which are potential oxidants that can cause tissue damage at the site of inflammation. Metronidazole for external use is ineffective against telangiectasias observed in rosacea.
Aerobic microorganisms and facultative anaerobes are insensitive to metronidazole, but in the presence of mixed flora (aerobes and anaerobes), metronidazole acts synergistically with antibiotics that are effective against conventional aerobes.
Indications
Rosacea (including post-steroidal acne), vulgar acne, infectious skin diseases, trophic ulcers of the lower extremities (against the background of varicose veins, diabetes mellitus), sluggish wounds, bedsores, burns, eczema, seborrheic eczema, oily seborrheic dermatitis, hemorrhoids, anal fissures.
Contraindications
Hypersensitivity to the components that make up the drug, as well as to nitroimidazole derivatives.
Side effects
When applied topically, the concentration of metronidazole in the blood is very low, so the risk of developing systemic side effects is low. Rarely may occur: allergic reactions (skin rash, itching, urticaria); hyperemia, peeling and burning of the skin, lacrimation (if the gel is applied close to the eyes).
Interaction
When used concomitantly, it enhances the effect of warfarin and other indirect anticoagulants (increases prothrombin time).
How to take, course of use and dosage
For external use. Apply to pre-cleansed skin in a thin layer 2 times a day, morning and evening, for 3-9 weeks. If necessary, apply an occlusive dressing. The average duration of the course of treatment is 3-4 months, the therapeutic effect is usually noted after 3 weeks of treatment
Overdose
There were no cases of overdose of the drug when prescribed at the recommended doses.
Special instructions
It is not recommended to prescribe the drug to pregnant women in the first trimester of pregnancy.
During lactation, if it is necessary to prescribe the drug, the question of stopping breastfeeding should be decided.
