Pharmacological action
The Active ingredient of Norcolut-norethisterone is mainly a progestogen-type norsteroid. Compared to other known progestogens, although the drug is not one of the most active progestogens, its properties are still unusually favorable for therapy.
In the so-called “delay of mensis” test, which is used to determine the effectiveness of progestogens, the drug exhibits the properties of weak progestogens. The compound proved to be a moderate-intensity progestive steroid in the Hammerstein test, as well as in the Clauberg test, which measures the dose required for endometrial transformation.
In contrast to many progestogenic compounds, norethisterone also has a pronounced anti-estrogenic character, and unlike other nortestosterone derivatives, only a small part of the compound is converted to estrogen in the body.
At the same time, Paulsen under the influence of norethisterone observed gynecomastia in men, and breast enlargement in women. In addition to the main progestogenic action, a weak androgenic property of the compound was also determined. According to clinical and pharmacological studies, norethisterone blocks the production of follicle-stimulating hormone (FSH) and yellow body hormone (LH), reduces the synthesis of steroids by the yellow body.
Norethisterone, which has mainly a progestogenic property, shows a significant anti-estrogenic and minimal estrogenic and androgenic effect, can be very widely used in the field of obstetrics and gynecology.
Indications
Premenstrual syndrome, mastodynia, cycle disorders accompanied by a shortened secretion period, dysfunctional uterine bleeding, endometrial cystic glandular hyperplasia, endometriosis, adenomyosis, stopping and preventing lactation, postponing menstruation, menopause, in case of individual need, oral contraception.
Contraindications
Predisposition to thrombosis, hepatitis, disorders of liver function, malignant tumors of the breast and genitals.
Side effects
Rarely encountered and passing in further cycles: nausea, gastrointestinal complaints, extraordinary bleeding, sometimes edema, allergic skin reaction, paresthesia, weight change, fatigue.
Interaction
Drugs that activate microsomal liver enzymes (barbiturates, etc. ) accelerate metabolism and weaken the effect;
on the contrary, inhibitors (cimetidine, etc. ) enhance it.
How to take, course of use and dosage
In the case of premenstrual syndrome, mastodynia, as well as promiscuous cycles,1-2 tablets are prescribed from the 16th day of the cycle to the 25th day of the cycle; this dosage can be supplemented with estrogen.
In case of dysfunctional uterine bleeding, cystic glandular endometrial hyperplasia (if the functional origin of the bleeding is confirmed by histological analysis performed in a 6-month period),1-2 tablets a day are prescribed to stop the bleeding for 6-12 days. To prevent relapse, starting from the 16th day of the cycle to the 25th day of the cycle,1-2 tablets a day are prescribed, usually together with estrogen. In the case of a malignant process, Norcolut therapy is not used.
In the case of endometriosis, adenomyosis, starting from the 5th day and ending with the 25th day of the cycle,1 tablet per day is used for 6 months or continuously starting from the 5th day of the cycle,1/2 tablet per day, increasing the dose by 1/2 tablet every 2-3 weeks, for 4-6 months.
To stop lactation on days 1-3,4 tablets each, on days 4-7,3 tablets each, and on days 8-10,2 tablets each. For menopausal symptoms,1 tablet per day is prescribed for a long time (for months), or, in case of failure, it is used in combination with 25-50 mcg of ethinyl estradiol. For the purposes of oral contraception,1/4—1/2 tablets of Norcolut and 0.05 mg of ethinyl estradiol are used starting from the 5th day of the cycle for 21 days, then after a 7-day break, the course is repeated. If necessary, a 2-phase contraceptive dosage is prescribed from the 5th day of the cycle for 10 days for 1/4 tablet of Norcolut, then for 11 days 1/2 tablet of Norcolut and 0.05 mg of ethinyl estradiol.
To delay menstruation, treatment begins 8 days before the expected menstruation and continues for 12 days: at the same time,2 tablets of Norcolut are prescribed per day, this dosage provides approximately 7-8 days of postponement of menstruation.
