Pharmacological action
Pancreatin Forte is a digestive enzyme agent that compensates for the lack of secretory function of the pancreas and biliary function of the liver, improves food digestion, has proteolytic, amylolytic and lipolytic effects.
Included in the pancreatic enzymes (lipase, alpha-amylase, trypsin, chymotrypsin) promote the breakdown of proteins to amino acids, fats to glycerol and fatty acids, starch to dextrins and monosaccharides.
Trypsin suppresses the stimulated secretion of the pancreas, having an analgesic effect.
The enzyme hemicellulase promotes the breakdown of plant fiber, which also improves digestive processes, reduces the formation of gases in the intestines. Bile extract acts as a choleretic agent, promotes fat emulsification, improves the absorption of fats and fat-soluble vitamins, and increases lipase activity.
Pharmacokinetics
Pancreatic enzymes are released from the dosage form in the alkaline environment of the small intestine, because they are protected from the action of gastric juice by an enteric-soluble membrane.
The maximum enzymatic activity of the drug is noted in 30-45 minutes after oral use.
Indications
Substitution therapy for external secretory pancreatic insufficiency: chronic pancreatitis, pancreatectomy, condition after radiation exposure, dyspepsia, cystic fibrosis; flatulence, diarrhea of non-infectious origin.
Violation of food assimilation (condition after resection of the stomach and small intestine); to improve food digestion in people with normal gastrointestinal function in case of errors in nutrition (consumption of fatty foods, large amounts of food, irregular meals) and with violations of chewing function, sedentary lifestyle, prolonged immobilization. Remheld’s syndrome (gastrocardiac syndrome).
Preparation for X-ray examination and ultrasound examination of the abdominal organs.
Contraindications
Hypersensitivity (including pork intolerance), acute pancreatitis, exacerbation of chronic pancreatitis.
Side effects
Allergic reactions, rarely-diarrhea or constipation, nausea, epigastric discomfort, abdominal pain, decreased endogenous bile acid synthesis. With prolonged use in high doses – hyperuricosuria, when using high doses in patients with cystic fibrosis-strictures in the ileocecal region and in the ascending colon, manifested by symptoms of intestinal obstruction.
Interaction
When used concomitantly with Pancreatin, the absorption of paraaminosalicylic acid, sulfonamides, and antibiotics increases. Reduces iron absorption (especially with prolonged use). Antacids containing calcium carbonate and / or magnesium hydroxide may reduce the effectiveness of Pancreatin.
How to take, course of use and dosage
Inside, by swallowing whole (without chewing), during or immediately after a meal, with plenty of liquid (if possible alkaline: water, fruit juices). The dose of the drug is set individually (in terms of lipase), depending on the age and degree of pancreatic insufficiency.
It is not recommended to exceed the maximum daily dose of enzymes of 15,000-20,000 units of lipase/kg, especially in patients with cystic fibrosis.
The average dose for adults is 150 thousand rubles. U/day; with complete insufficiency of the external secretory function of the pancreas – 400 thousand rubles. U/day, which corresponds to the daily need of an adult for lipase.
The duration of treatment can vary from a single dose or several days (if the digestive process is disrupted due to errors in the diet) to several months or even years (if permanent replacement therapy is required). Adults – 3-4 tablets 3 times a day. Higher doses are prescribed by your doctor. Take 2 tablets 2-3 times a day for 2-3 days before X-ray or ultrasound examination.

Overdose
Symptoms: With prolonged use in high doses-hyperuricosuria, when using high doses in patients with cystic fibrosis-strictures in the ileocecal region and in the ascending colon. Hyperuricemia. Children have constipation. Treatment: drug withdrawal, symptomatic therapy.
Special instructions
In cystic fibrosis, the drug should be administered with caution because the dose should be adequate to the quantity and quality of food consumed. With prolonged use in cases of digestive disorders (with normal pancreatic function), iron preparations should be prescribed simultaneously.
