Composition
Active ingredient-azamethonium bromide.
Pharmacological action
Ganglioblocker, a bisquaternary ammonium compound.
It has antihypertensive, arterio-and venodilating effects. It blocks the n-holinoreceptors of the autonomic ganglia and therefore inhibits the transmission of nervous excitation from preganglionic to postganglionic fibers of the autonomic nerves. Vegetative ganglia also become insensitive to the excitatory action of a number of cholinergic stimuli (including acetylcholine, nicotine, lobelin, cytisine).
Interrupting the conduction of nerve impulses through the autonomic ganglia, ganglioblockers alter the functions of organs equipped with vegetative innervation. At the same time, there is a decrease in blood pressure, which is mainly associated with a decrease in the flow of vasoconstrictor impulses to the blood vessels and the expansion of the peripheral vascular bed (primarily arterioles).
Inhibition of the conduction of impulses along cholinergic nerve fibers leads to impaired accommodation, expansion of the bronchi, decreased motility of the gastrointestinal tract, inhibition of glandular secretion, increased heart rate, and decreased bladder tone.
Inhibition of n-cholinergic receptors in the adrenal chromaffin tissue leads to a decrease in the release of adrenergic substances and a weakening of reflex pressor reactions. In high doses, it can block the n-cholinergic receptors of skeletal muscles and the central nervous system. Azamethonium bromide is somewhat inferior to benzohexonium in terms of activity and duration of action.
Indications
Diseases and conditions caused by peripheral vascular spasms (including endarteritis, intermittent claudication). As part of combination therapy for hypertensive crisis, pulmonary edema, brain edema.
In anaesthetic practice – for controlled arterial hypotension.
In urological practice – during cystoscopy in men (to facilitate the passage of the cystoscope through the urethra).
Use during pregnancy and lactation
There are no data on the safety of using azamethonium bromide during pregnancy and lactation (breastfeeding).
Contraindications
Angle-closure glaucoma, acute phase of myocardial infarction, severe hypotension, shock, impaired liver and/or kidney function, pheochromocytoma, degenerative diseases of the central nervous system, hypersensitivity to azamethonium bromide.
Side effects
Dry mouth, weakness, dizziness, tachycardia, mydriasis, orthostatic hypotension.
Interaction
The antihypertensive effect of the drug decreases when combined with tricyclic antidepressants, mezaton, ephedrine; increases-with non-depolarizing muscle relaxants, other antihypertensive agents,
How to take, course of use and dosage
The drug is administered intramuscularly and intravenously.
In case of vascular spasms and other nonextensive conditions, the drug is administered intramuscularly, in an initial dose 1 ml of a 5% solution, if necessary, increase the dose to 1.5-2 ml 2-3 times a day. The highest single dose for adults is 0.15 g (3 ml of 5% solution), the daily dose is 0.45 g.
In hypertensive crises, pulmonary edema, brain edema, enter / in slowly (under the control of blood pressure) 0.2-0.5 ml of 5% solution, diluted in 20 ml of isotonic sodium chloride solution or 5% glucose solution or in / m to 2 ml of 5% solution.
For controlled arterial hypotension,40-60 mg (0.8 – 1.2 ml of 5% solution) is administered intravenously before surgery, if necessary – 120-180 mg (2.4 – 3.6 ml of 5% solution).
Special instructions
To prevent the development of orthostatic hypotension after use of the drug, patients should be in the supine position for 1-2 hours.
