Composition
100 g:
– betamethasone dipropionate in terms of betamethasone 0.05 g
-salicylic acid 3.0 g
excipients:
liquid paraffin (vaseline oil),
vaseline (white vaseline, soft white paraffin).
Pharmacological action
A drug with anti-inflammatory and keratolytic effects for external use.
Indications
Treatment of subacute and chronic dermatoses accompanied by hyperkeratosis and peeling:
— psoriasis;
— skin dermatitis;
eczema;
— atopic dermatitis;
— lichen planus;
— ichthyosis and ihtioze lesions of the skin;
atopic dermatitis,
diffuse neurodermatitis;
— simple and allergic dermatitis;
— urticaria;
— exudative erythema multiforme;
— simple chronic lichen (limited neurodermatitis);
— dermatitis, is not amenable to treatment with other corticosteroids (especially red warty lichen);
— dishidros skin.
Contraindications
— open wounds;
bacterial, viral and fungal skin diseases (pyoderma, syphilis, tuberculosis of the skin, chickenpox, herpes, actinomycosis, blastomycosis, sporotrichosis);
— post-vaccination skin reactions;
perioral dermatitis;
— rosacea;
— trophic ulcers in chronic venous insufficiency;
— skin tumors (cancer of skin, nevus, sebaceous cyst, epithelioma, melanoma, hemangioma, a xanthoma, sarcoma);
— children’s age up to 2 years;
— lactation period;
— hypersensitivity to the components of the drug.
The drug should be used with caution in patients with hepatic insufficiency, in children over 2 years of age, for long-term therapy, especially with the use of occlusive dressings.
Side effects
-Burning, irritation, dryness, hypopigmentation, itchy skin, folliculitis, hypertrichosis, dermatitis, acne-like rashes. – When using occlusive dressings – maceration of the skin, striae, skin atrophy, secondary infection, sweating. – With prolonged use-skin atrophy, local hirsutism, telangiectasia, purpura, hypopigmentation, hypertrichosis.
– When applied to large surface – systemic manifestations of side effects of corticosteroids (stunting syndrome Itsenko-Kushinga, increased intracranial pressure after treatment, hyperglycemia, glucosuria, hypokalemia, increased blood pressure), or salicylates (pallor, fatigue, drowsiness, hyperventilation on the background of tachypnea, nausea, vomiting, hearing impairment, confusion).
How to take, course of use and dosage
Externally. Apply to the affected areas at the rate of: a column of Rederm ointment 0.5 cm long on a skin area of approximately 7 cm×7 cm, lightly rubbing, in a thin layer 2 times a day.
Another frequency of use can be determined by the doctor, based on the severity of the disease. In mild cases, it is sufficient to apply the ointment, as a rule, once a day; with more severe lesions, more frequent use may be required.
If necessary, apply occlusive dressings, which are changed every 24 hours.
The duration of treatment depends on the effectiveness and tolerability of the therapy and is 2-4 weeks.
If clinical improvement does not occur, the diagnosis should be clarified.
Overdose
Acute overdose is unlikely, however, with excessive or prolonged use of the drug, a chronic overdose is possible, accompanied by signs of hypercorticism: hyperglycemia, glucosuria, reversible suppression of the adrenal cortex, manifestation of Cushing’s syndrome.
Treatment.
Appropriate symptomatic treatment is indicated. In case of chronic toxic effects, gradual discontinuation of the drug is recommended.
Special instructions
Avoid contact with the eyes, do not apply on the skin near the eyes and on the mucous membranes. The drug is not intended for use in ophthalmology. Systemic absorption of glucocorticosteroids is possible with prolonged treatment, when the drug is applied to extensive skin surfaces, and when occlusive dressings are used.
Long-term use on the face is not recommended – rosacea-type dermatitis, perioral dermatitis, skin atrophy and acne may develop; avoid use in the anogenital area.
Rederm ointment is most suitable for the treatment of patients with dry and fragile skin.
If there are signs of hypersensitivity or skin irritation associated with the use of the drug, treatment should be discontinued and appropriate therapy should be selected for the patient.
With prolonged use of the drug, its cancellation should be carried out gradually.
Application in pediatrics:
Children from 2 years of age are prescribed the drug only for strict indications and under medical supervision, as it is possible to develop systemic side effects associated with betamethasone. The duration of treatment should be as short as possible.
When using the drug Rederm on extensive surfaces and/or under an occlusive dressing, it is possible to suppress the function of the hypothalamic-pituitary-adrenal system and develop symptoms of hypercorticism, there may be a decrease in the excretion of growth hormone, an increase in intracranial pressure.
