Pharmacological action

Pharmacodynamics

Mucolytic and expectorant action of Fluditek is caused by activation of sialic transferase, an enzyme of goblet cells of the bronchial mucosa. Normalizes the quantitative ratio of acidic and neutral sialomucins of bronchial secretions, restores the viscosity and elasticity of mucus. Promotes regeneration of the mucous membrane, normalizes its structure, activates the activity of the ciliated epithelium. Restores the secretion of immunologically active IdA (specific protection) and the number of sulfhydryl groups of mucus components (non-specific protection), improves mucociliary clearance.

Pharmacokinetics

After oral use, Cmax in the blood serum and in the respiratory mucosa is reached in 2-3 hours and persists in the mucosa for 8 hours. It is mainly excreted by the kidneys, partly in unchanged form, partly in the form of metabolites.

Indications

— acute and chronic bronchopulmonary diseases associated with formation of a viscous, difficult to spit phlegm (tracheitis, bronchitis, tracheobronchitis, bronchial asthma, bronchiectasis);

— acute and chronic inflammatory diseases of the nasal cavity, nasopharynx, paranasal sinuses and middle ear, accompanied by the formation of a viscous, difficult to spit slime (rhinitis, adenoiditis, sinusitis, otitis media);

— preparation for bronchoscopy and/or bronhografii.

Use during pregnancy and lactation

Contraindicated in the first trimester of pregnancy.

Contraindications

-peptic ulcer of the stomach and duodenum in the acute phase; – chronic glomerulonephritis( in the acute phase), cystitis;— children under 2 years of age (for syrup 20 mg / ml for children) and up to 15 years (for syrup 50 mg / ml)— – I trimester of pregnancy (for syrup 50 mg / ml);- hypersensitivity to carbocysteine or other components of Fluditek.

With caution, the drug should be prescribed for chronic glomerulonephritis( in the anamnesis), peptic ulcer of the stomach and duodenum (in the anamnesis).

Side effects

From the digestive tract: sometimes — nausea, vomiting, diarrhea, epigastric pain, gastrointestinal bleeding.

Allergic reactions: in isolated cases — itching, urticaria, exanthema, angioedema.

Nervous system and sensory disorders: dizziness, weakness, malaise.

Interaction

When combined with corticosteroids, a synergistic effect is noted.

Increases the effectiveness of antibacterial therapy of inflammatory diseases of the upper and lower respiratory tract.

Enhances the bronchodilator effect of theophylline.

Carbocysteine activity is weakened by antitussive and atropine-like agents.

How to take, course of use and dosage

For children over 5 years of age, the daily dose is 300 mg, i. e. 1 teaspoon (5 ml) 3 times a day.

Children from 2 to 5 years – 200 mg/day, i. e. 1 teaspoon 2 times / day.

Newborns and children from 1 month to 2 years – 20-30 mg / kg / day in 1 or 2 doses, while not exceeding the dose of 100 mg/day.

The duration of treatment, as a rule, should not exceed 8-10 days.

Overdose

Symptoms: gastralgia, nausea, diarrhea.

Treatment: symptomatic therapy.

Special instructions

Fluditec should be used with caution when coughing wetly.

People with peptic ulcer of the stomach and duodenum are recommended to prescribe the drug with caution.

Simultaneous use of Fluditek with antitussive drugs and/or substances that inhibit bronchial secretion (including atropine-like agents) is not recommended.

When prescribing the drug for concomitant diabetes mellitus, as well as for patients on a low-sugar diet, the sucrose content (3.5 g per teaspoon in syrup for children) should be taken into account.
