Composition
of 1 tab. : acetylsalicylic acid 100 mg. Auxiliary substances: colloidal silicon dioxide, lactose, microcrystalline cellulose, potato starch, talc, triacetin, silicone anti-foaming agent SE2, eudrazhit L30D (methacrylic acid and ethacrylate copolymer).
Pharmacological action
The drug Trombo Acc-NSAIDs, antiplatelet agent. Reduces blood clotting due to a decrease in platelet aggregation by suppressing the synthesis of thromboxane A2 in them. The use of Thrombo ASS, starting with a dose of 50 mg, leads to a prolonged 90-95% decrease in the serum concentration of thromboxane A2 and its metabolites. It also has anti-inflammatory, analgesic and antipyretic effects. Tablets of the drug are coated with a film-like enteric coating that is resistant to the action of gastric juice, which reduces the risk of side effects from the stomach.
Indications
-prevention of deep vein thrombosis and thromboembolism of the pulmonary artery and its branches (for example, with prolonged immobilization as a result of serious surgical intervention). – prevention of acute myocardial infarction in the presence of risk factors (such as diabetes mellitus, hyperlipidemia, arterial hypertension, obesity, smoking, old age);- secondary prevention of myocardial infarction;- unstable angina pectoris— – stroke prevention (including in patients with transient cerebral circulatory disorders);— prevention of transient cerebral circulatory disorders— – prevention of thromboembolism after surgery and invasive vascular interventions (such as aorto-coronary bypass surgery, carotid artery endarterectomy, arterio-venous bypass surgery, carotid artery angioplasty);
Contraindications
— I and III trimesters of pregnancy;- lactation (breastfeeding);— children and adolescents under 18 years of age; – erosive and ulcerative lesions of the gastrointestinal tract, gastrointestinal bleeding;- “aspirin” asthma (bronchial asthma induced by taking salicylates and NSAIDs);- “aspirin triad “(a combination of bronchial asthma, recurrent nasal and paranasal sinus polyposis, and acetylsalicylic acid intolerance);- hemorrhagic diathesis;- combined use with methotrexate at a dose of 15 mg or more per week;- hypersensitivity to acetylsalicylic acid, to excipients of the drug Prombo ACC and other NSAIDs. Use with caution for gout, hyperuricemia, ulcerative lesions of the gastrointestinal tract or bleeding from the gastrointestinal tract in the anamnesis, with renal and hepatic insufficiency, bronchial asthma, chronic respiratory diseases, hay fever, nasal polyposis, allergic reactions to medications in the anamnesis, when used simultaneously with methotrexate at a dose of 15 mg or less per week.
Side effects
From the digestive system: nausea, heartburn, vomiting, abdominal pain, ulcers of the gastric and duodenal mucosa (including perforated ones), increased activity of liver enzymes. From the respiratory system: bronchospasm. From the hematopoietic system: anemia (rarely), increased bleeding. From the central nervous system: dizziness, tinnitus. Allergic reactions: urticaria, angioedema, anaphylactic reactions. In general, Thrombo ASS® is well tolerated by patients (due to the low content of acetylsalicylic acid in the preparation). Side effects are noted in some cases.
Interaction
When used concomitantly, Trombo ACC® enhances the effect of the following medications: : – thrombolytic and antiplatelet agents (ticlopidine);- digoxin due to a decrease in its renal excretion— – hypoglycemic agents (insulin and sulfonylurea derivatives) due to the hypoglycemic properties of ASA itself in high doses and the displacement of sulfonylurea derivatives from protein bonds— – valproic acid due to its displacement from protein bonds. — methotrexate due to a decrease in renal clearance and its displacement from protein binding; – heparin and indirect anticoagulants due to impaired platelet function and displacement of indirect anticoagulants from protein binding;An additive effect is observed with simultaneous use of ASA with ethanol. ASA weakens the effect of uricosuric agents (benzbromarone) due to competitive tubular elimination of uric acid. When used simultaneously, corticosteroids increase the excretion of salicylates and weaken their effect.
How to take, course of use and dosage
Inside, Tablets of Trombo ACC, without chewing, before eating, with a small amount of liquid. Assign inside 50 -100 mg 1 time/day. Trombo ACC® is intended for long-term use. The duration of therapy is determined individually.
Overdose
Overdose is especially dangerous in elderly patients and is unlikely due to the low content of ASA in the drug. Excess of the ASA dose is associated with the risk of gastrointestinal bleeding. Symptoms: consciousness, nausea, vomiting, tinnitus, dizziness, confusion and general malaise. Treatment: artificial vomiting, use of activated charcoal, laxatives; if necessary, correction of the acid-base balance is carried out.
Special instructions
When combined with corticosteroids, it should be remembered that during treatment, the level of salicylates in the blood is reduced, and after the withdrawal of corticosteroids, an overdose of salicylates is possible. The combination of ASA with ibuprofen is not recommended, as the latter worsens the beneficial effect of ASA on life expectancy. When ASA is combined with alcohol, the risk of damage to the gastrointestinal mucosa and prolongation of bleeding time is increased. ASA can provoke bronchospasm, as well as cause attacks of bronchial asthma and other hypersensitivity reactions. Risk factors include a history of bronchial asthma, hay fever, nasal polyposis, chronic respiratory diseases, and allergic reactions to other medications (pruritus, urticaria). ASA can cause bleeding of varying severity during and after surgery. The combination of ASA with anticoagulants, thrombolytics, and antiplatelet agents is associated with an increased risk of bleeding. ASA in low doses can provoke the development of gout in predisposed individuals with reduced uric acid excretion. The combination of ASA with methotrexate is accompanied by an increased frequency of side effects from the hematopoietic system. High doses of ASA have a hypoglycemic effect, which should be taken into account when prescribing it to patients with diabetes mellitus receiving hypoglycemic drugs.
