Clinical Pharmacology
Pharmacodynamics : Tetrisoline is a sympathomimetic drug that stimulates the alpha-adrenergic receptors of the sympathetic nervous system, but has no or weak effect on beta-adrenergic receptors. Being a sympathomimetic amine, tetrisoline has a vasoconstrictive effect and reduces tissue edema. The effect begins 60 seconds after instillation and lasts 4-8 hours. Pharmacokinetics: When applied topically, it is practically not absorbed. Detailed pharmacokinetic studies after topical application of eye drops have not been conducted.
Indications
To relieve swelling and hyperemia of the conjunctiva (redness of the eyes) caused by exposure to chemical and physical factors (smoke, wind, dust, chlorinated water, light, cosmetics, contact lenses), as well as arising from allergic reactions, such as pollinosis.
Use during pregnancy and lactation
Data on the penetration of the drug through the placenta and into breast milk are not available. Taking into account the possibility of developing systemic side effects, the drug can be used during pregnancy and lactation only if, in the opinion of the doctor, the expected benefit to the mother exceeds the potential risk to the fetus and child.
Contraindications
- hypersensitivity to the components of the drug;
- angle-closure glaucoma;
- children under 2 years of age.
Side effects
Post-marketing dataoften (>1/10), often (>>1/100, >><1/10), infrequently (>1/1000, <1/10), infrequently (><1/100), rarely (>1/10000, <1/100), rarely (><1/1000), very rarely (Visual disturbances: dilated pupil (frequency unknown). General disorders and disorders at the injection site: reactions at the injection site (including burning sensation in the eye area, redness, irritation, swelling, pain, itching) (frequency unknown). It is assumed that the frequency, type and severity of adverse reactions in children should be the same as in adults.
Interaction
The drug “Visin ® Classic” should not be used simultaneously with MAO inhibitors such as tranylcypromine, tricyclic antidepressants, as well as drugs that increase blood pressure. Combined therapy with these groups of drugs can lead to increased vasoconstrictor action and increased blood pressure.
How to take, course of use and dosage
Locally. Adults and children over 2 years of age: 1 drop in the affected eye 2-3 times a day. The use of the drug for more than 4 days should be carried out only under the supervision of a doctor.
Overdose
When used in accordance with the instructions, the risk of overdose is minimal. However, if the drug accidentally enters the gastrointestinal tract (ingestion), the following symptoms of overdose may occur: pupil dilation, nausea, cyanosis, fever, convulsions, tachycardia, arrhythmia, cardiac arrest, hypotension, pulmonary edema, respiratory depression, including apnea (respiratory arrest), central nervous system depression, including drowsiness and coma. The risk of overdose symptoms due to systemic effects of the drug is high in newborns and young children, especially if swallowed. The specific antidote is unknown. Treatment of overdose in case of ingestion: prescribe activated charcoal, gastric lavage, oxygen inhalation, antipyretics and anticonvulsants. To reduce blood pressure, phentolamine 5 mg in saline solution is used slowly intravenously or 100 mg orally. Vasopressors are contraindicated in patients with low blood pressure. If you experience any symptoms of overdose, you should immediately consult a doctor!

Special instructions
Visin ® Classic should be used with caution in elderly patients, in patients with aneurysms, hypertension and / or coronary heart disease, as well as in patients with type I diabetes mellitus or hyperthyroidism. Safety studies of the drug in children and adolescents have not been conducted. Contact lenses should be removed before instillation of the drug and installed after 15 minutes. It is necessary to avoid direct contact of the drug with soft contact lenses because of the possible violation of their transparency. Possible development of reactive hyperemia of the conjunctiva and nasal mucosa (drug-induced rhinitis) if you do not follow the instructions for use. If the condition does not improve within 72 hours, or if the irritation and redness persist or increase, discontinue the drug and consult a doctor. If you experience intense eye pain, severe acute or unilateral redness of the eyes, headache, visual disturbances, spots in front of the eyes or double vision, you should immediately consult a doctor. Prolonged use of the drug may increase hyperemia or lead to its reappearance. If the irritation or redness is caused by diseases of the visual organ: infection, foreign body or chemical injury of the cornea, consult a doctor before use. The use of the drug may cause temporary dilation of the pupil. Long-term use and overdose of the drug should be avoided, especially in children. Do not use the product if it changes color or becomes cloudy. Topical application of sympathomimetic amines in the conjunctival sac, as a result of pupil dilation, can sometimes lead to impaired intraocular pressure in predisposed individuals. If the medicine has become unusable or the expiration date has expired-do not pour it into the waste water and do not throw it on the street! Place the medicine in a plastic bag and put it in the trash can. These measures will help protect the environment!
