Pharmacological action
The action of epinephrine is associated with the effect on a and b – adrenergic receptors and largely coincides with the effects of excitation of sympathetic nerves:

It narrows the vessels of the abdominal cavity, skin, mucous membrane and, to a lesser extent, the vessels of skeletal muscles;

Adrenaline increases blood pressure, increases and accelerates heart contractions;

Due to an increase in blood pressure, there is a reflex excitation of the vagus nerve center, which has an inhibitory effect on the heart, as a result of which the heart rate may slow down.
Epinephrine can cause cardiac arrhythmias (tachycardia, extrasystole), especially in patients with coronary heart disease, as well as during anesthesia during operations.
Adrenaline relaxes the muscles of the bronchi and intestines, dilates the pupils, improves the functional activity of skeletal muscles, increases blood sugar, increases tissue metabolism, and increases the need for oxygen in the myocardium.
By the action of epinephrine, hydrotartrate does not differ from epinephrine hydrochloride, but due to the difference in relative molecular weight, hydrotartrate is used in a higher concentration to obtain the same effect.
Indications
Epinephrine hydrochloride is used for:
- Anaphylactic shock, allergic laryngeal edema, for relief of acute attacks of bronchial asthma;
- Allergic reactions that develop with the use of medications or other allergens,
- In patients with acute myocardial infarction with ventricular fibrillation, refractory to electrical defibrillation, and with sudden cardiac arrest (asystole);
- In acute left ventricular failure; as a local vasoconstrictor.
Contraindications
Epinephrine is contraindicated in hypertension, aneurysms, severe atherosclerosis, bleeding, pregnancy.
Epinephrine should not be used for anesthesia with fluorotane, cyclopropane, or chloroform (risk of arrhythmia).
Use with caution in patients with diabetes mellitus, hyperthyroidism.
Side effects
use of epinephrine hydrochloride and hydrotartrate may cause increased blood pressure, tachycardia, arrhythmia, and heart pain.
In case of rhythm disturbances caused by epinephrine, beta-blockers (anaprilin, obzidan, etc. ) are prescribed.
Interaction
Enhances the effect of Euphylline and drugs that increase thyroid function.
How to take, course of use and dosage
Epinephrine is administered subcutaneously, intramuscularly, intravenously (drip slowly).0,2 – 0,3 – 0,5 – 1 1 ml of 0.1% hydrochloride solution, intracardiac with acute cardiac arrest-1 ml and with ventricular fibrillation-0.5-1 ml, and also applied to the mucous membranes as a local vasoconstrictor.
During an attack of bronchial asthma, epinephrine solutions are administered by 0,3 – 0,5 – 0,7 ml subcutaneously.
Therapeutic doses of epinephrine hydrochloride solutions 0.1% for parenteral use are usually for adults 0,3 – 0,5 – 0,75 children, depending on their age, are given 0.1-0.5 ml. The highest doses for adults under the skin: single – 1 ml, daily-5 ml.
