Pharmacological action

A combined remedy, the action of which is determined by the components that make up its composition. It has an antacid, adsorbent, enveloping, carminative effect.

Algeldrate (aluminum hydroxide) and magnesium hydroxide neutralize free hydrochloric acid in the stomach, reduce the acidity of gastric juice, and bind bile acids.

The laxative effect of magnesium hydroxide balances the ability of algeldrate to slow intestinal motility.

Simethicone hinders the formation of gas bubbles and contributes to their destruction. The gases released in this case are absorbed by the intestinal walls and removed from the body due to peristalsis.

Pharmacokinetics

Simethicone due to physiological and chemical inertia is not absorbed into organs and tissues and after passing through the gastrointestinal tract (GIT) is excreted unchanged.

The absorption of aluminum and magnesium ions in the intestine is low.

With normal kidney function, the concentration of aluminum and magnesium in the blood does not change.

In patients with chronic renal failure, the level of aluminum and magnesium in the blood may increase to toxic values as a result of impaired excretion.

Indications

Acute gastritis;chronic gastritis with increased and normal secretory function of the stomach (in the acute phase);acute duodenitis, duodenogastric reflux;peptic ulcer of the stomach and duodenum (in the acute phase);symptomatic ulcers of the gastrointestinal tract of various origins;erosion of the mucous membrane of the upper gastrointestinal tract;gastroesophageal reflux, reflux esophagitis;acute pancreatitis, exacerbation of chronic pancreatitis;gastralgia, heartburn (after excessive use of ethanol, nicotine, coffee, taking medications;incorrect diet, which negatively affects the functioning of the gastrointestinal tract);flatulence;fermentation or putrefactive dyspepsia.

Use during pregnancy and lactation

It is not recommended to use the drug Almagel Neo during pregnancy and lactation (breastfeeding).

Contraindications

Hypersensitivity, chronic renal failure, pregnancy, Alzheimer’s disease, hypophosphatemia, children under 10 years of age, congenital fructose intolerance.

With caution:

Lactation period, liver diseases, alcoholism, traumatic brain injury, brain diseases, epilepsy, children and adolescents from 10 to 18 years.

Side effects

Allergic reactions, nausea, vomiting, changes in taste sensations, constipation, diarrhea.

When taken for a long time in high doses:  hypophosphatemia, hypocalcemia, hypercalciuria, osteomalacia, osteoporosis, hypermagnesemia, hyperaluminemia, encephalopathy, nephrocalcinosis, impaired renal function.

In patients with concomitant renal insufficiency:  thirst, low blood pressure, hyporeflexia.

Interaction

Almagel ® Neo reduces and slows down the absorption of digoxin, Indometacin, salicylates, chlorpromazine, phenytoin, H2-histamine receptor blockers, beta-blockers, diflunisal, ketoconazole and itraconazole, isoniazid, tetracycline-type antibiotics and quinolones, azithromycin, cefpodoxime, pivampicillin, rifampicin, indirect anticoagulants, barbiturates, fexofenadine, dipyridamole, zalcitabine, chenodeoxycholic and efsodeoxycholic acids, penicillamine and lansoprazole. M-holinoblokatory, slowing down the emptying of the stomach, enhance and lengthen the effect of the drug.

How to take, course of use and dosage

Adults

Inside, take 2 scoops or 1 sachet of orange-flavored suspension 4 times a day 1 hour after meals and in the evening before going to bed. If necessary, a single dose can be increased to 3 scoops 4 times a day. The maximum daily dose should not exceed 12 scoops.

Children over 10 years of age

The dosage is determined by the attending physician-usually 1/2 dose for adults. The course of treatment is no more than 4 weeks. Before use, the suspension must be thoroughly homogenized by shaking the bottle or kneading and shaking the sachet.

It is advisable to take Almagel ® Neo without diluting it with water or washing it down. It is not recommended to take fluids within half an hour after taking the drug.

Overdose

Symptoms.

Prolonged use of high doses can lead to the development of hypermagnesemia, which is characterized by rapid fatigue, redness of the face, exhaustion, muscle weakness and inappropriate behavior.

There may also be signs of metabolic alkalosis: changes in mood, impaired mental activity, numbness or pain in the muscles, nervousness and rapid fatigue, slow breathing, unpleasant taste sensations.

Urgent measures.

It is necessary to immediately take measures to quickly remove the drug – gastric lavage, stimulation of vomiting, taking activated charcoal.

Special instructions

Interval between Almagel®use Neo and other medications should be 1-2 hours long. When taking it for a long time, it is necessary to ensure sufficient intake of phosphorus with food. Due to the lack of data from controlled clinical trials, breast-feeding women should only take Almagel® Neo after consulting their doctor. One measuring spoon (5 ml) of the drug contains 0.113 g of ethyl alcohol. One sachet (10 ml) of the drug contains 0.226 g of ethyl alcohol. As a result, complications may occur in patients with liver and brain diseases, alcoholism and epilepsy, pregnant women and children under 18 years of age. The daily dose of the drug (8 scoops or 4 sachets) contains 0.904 g of ethyl alcohol, the maximum daily dose of the suspension (12 scoops or 6 sachets) contains 1.356 g of ethyl alcohol. One scoop (5 ml) of Almagel ® Neo contains 0.475 g of sorbitol. One sachet (10 ml) of the suspension contains 0.950 g of sorbitol. Sorbitol is contraindicated in patients with congenital fructose intolerance and may cause stomach irritation and diarrhea. Influence on the ability to drive vehicles and mechanisms: Almagel ® Neo does not adversely affect the ability to drive vehicles and work with mechanisms that require increased concentration of attention and speed of psychomotor reactions.
