Pharmacological Action

Antiviral drug – a synthetic analogue of an acyclic purine nucleoside, which has a highly selective effect on herpes viruses.

In virus-infected cells, under the action of viral thymidine kinase, phosphorylation and further sequential transformation of acyclovir into mono-, di- and triphosphate occurs.

Acyclovir triphosphate is incorporated into the viral DNA chain and blocks its synthesis through competitive inhibition of viral DNA polymerase.

In vitro, Acyclovir is effective against the herpes simplex virus – Herpes simplex types 1 and 2; against the Varicella zoster virus, which causes chickenpox and shingles; higher concentrations are required to inhibit the Epstein-Barr virus.

It is moderately active against cytomegalovirus.

In vivo, Acyclovir is therapeutically and prophylactically effective, primarily, in viral infections caused by Herpes simplex virus types 1 and 2.

It prevents the formation of new rash elements, reduces the likelihood of skin dissemination and visceral complications, accelerates crust formation, and reduces pain in the acute phase of herpes zoster.

Pharmacokinetics

After oral administration, the bioavailability is 15-30%, with dose-dependent concentrations sufficient for effective treatment of viral diseases being achieved.

Food does not significantly affect the absorption of acyclovir.

Acyclovir penetrates well into many organs, tissues and body fluids.

Plasma protein binding is 9-33% and does not depend on its plasma concentration.

The concentration in the cerebrospinal fluid is about 50% of its plasma concentration.

Acyclovir crosses the blood-brain barrier and placental barrier, and accumulates in breast milk.

After oral administration of 1 g/day, the concentration of acyclovir in breast milk is 60-410% of its plasma concentration (the infant receives acyclovir from breast milk at a dose of 0.3 mg/kg/day).

The Cmax of the drug in blood plasma after oral administration of 200 mg 5 times/day is 0.7 µg/ml, Cmin is 0.4 µg/ml; the time to reach Cmax in plasma is 1.5-2 hours.

It is metabolized in the liver to form a pharmacologically inactive compound, 9-carboxymethoxymethylguanine.

It is excreted by the kidneys through glomerular filtration and tubular secretion: about 84% is excreted by the kidneys unchanged, 14% in the form of a metabolite.

The renal clearance of acyclovir is 75-80% of the total plasma clearance.

The T1/2 in adults with normal renal function is 2-3 hours.

In patients with severe renal failure, T1/2 is 20 hours, during hemodialysis – 5.7 hours, while the plasma concentration of acyclovir decreases to 60% of the initial value.

Less than 2% of acyclovir is excreted from the body through the intestines.

Indications

  • Treatment of infections of the skin and mucous membranes caused by Herpes simplex viruses types 1 and 2, both primary and secondary, including genital herpes;
  • Prevention of exacerbations of recurrent infections caused by Herpes simplex types 1 and 2 in patients with normal immune status;
  • Prevention of primary and recurrent infections caused by Herpes simplex viruses types 1 and 2 in patients with immunodeficiency;
  • As part of complex therapy for patients with severe immunodeficiency: in HIV infection (AIDS stage, early clinical manifestations and advanced clinical picture) and in patients who have undergone bone marrow transplantation;
  • Treatment of primary and recurrent infections caused by the Varicella zoster virus (chickenpox, as well as shingles – Herpes zoster).

Dosage Regimen

The method of application and dosage regimen for a specific drug depend on its form of release and other factors. The optimal dosage regimen is determined by the doctor. It is necessary to strictly adhere to the compliance of the dosage form of a specific drug with the indications for use and dosage regimen.

Tablets

Acyclovir should be taken during or immediately after a meal and washed down with a sufficient amount of water.

The dosage regimen is set individually depending on the severity of the disease.

Treatment of infections of the skin and mucous membranes caused by Herpes simplex types 1 and 2

Adults

Acyclovir is prescribed 200 mg 5 times/day for 5 days with 4-hour intervals during the day and an 8-hour interval at night.

In more severe cases of the disease, the course of treatment can be extended as prescribed by a doctor up to 10 days.

As part of complex therapy for severe immunodeficiency, including the advanced clinical picture of HIV infection, including early clinical manifestations of HIV infection and the AIDS stage; after bone marrow transplantation or in case of impaired intestinal absorption, 400 mg 5 times/day is prescribed.

Treatment should be started as early as possible after the onset of infection; for relapses, Acyclovir is prescribed in the prodromal period or when the first elements of the rash appear.

Prevention of relapses of infections caused by Herpes simplex types 1 and 2 in patients with normal immune status

The recommended dose is 200 mg 4 times/day (every 6 hours) or 400 mg 2 times/day (every 12 hours).

In some cases, lower doses are effective – 200 mg 3 times/day (every 8 hours) or 2 times/day (every 12 hours).

Prevention of infections caused by Herpes simplex types 1 and 2 in patients with immunodeficiency.

The recommended dose is 200 mg 4 times/day (every 6 hours).

In case of severe immunodeficiency (for example, after bone marrow transplantation) or in case of impaired intestinal absorption, the dose is increased to 400 mg 5 times/day.

The duration of the prophylactic course of therapy is determined by the duration of the period of risk of infection.

Treatment of infections caused by the Varicella zoster virus (chickenpox)

Adults

Prescribe 800 mg 5 times/day every 4 hours during the day and with an 8-hour interval at night.

The duration of the course of treatment is 7-10 days.

Children

Prescribe 20 mg/kg 4 times/day for 5 days (maximum single dose 800 mg), children from 3 to 6 years old: 400 mg 4 times/day, over 6 years old: 800 mg 4 times/day for 5 days.

Treatment should be started at the very first signs or symptoms of chickenpox.

Treatment of infections caused by the Herpes zoster virus (shingles)

Adults

Prescribe 800 mg 4 times/day every 6 hours for 5 days.

Children over 3 years of age are prescribed the drug in the same dose as adults.

Treatment and prevention of infections caused by Herpes simplex types 1 and 2 in pediatric patients with immunodeficiency and normal immune status.

Children from 3 to 6 years old – 400 mg 4 times/day; over 6 years old – 800 mg 4 times/day.

A more accurate dose is determined at the rate of 20 mg/kg of body weight, but not more than 800 mg 4 times/day.

The course of treatment is 5 days.

There are no data on the prevention of relapses of herpes simplex virus infections and the treatment of herpes zoster in children with normal immunity.

For the treatment of children over 3 years old, 800 mg of acyclovir 4 times/day every 6 hours is prescribed (as for the treatment of adults with immunodeficiency).

In old age, the clearance of acyclovir in the body decreases in parallel with the decrease in creatinine clearance.

Patients taking large doses of the drug orally should receive sufficient fluids.

In case of renal failure, the issue of reducing the dose of the drug must be decided.

Caution should be exercised when prescribing acyclovir to patients with renal failure.

In such patients, oral administration of the drug in recommended doses for the treatment and prevention of infections caused by the herpes simplex virus does not lead to the accumulation of the drug to concentrations exceeding established safe levels.

However, in patients with severe renal failure (creatinine clearance less than 10 ml/min), the dose of acyclovir should be reduced to 200 mg 2 times/day with 12-hour intervals.

When treating infections caused by the Varicella zoster virus, Herpes zoster, as well as when treating patients with severe immunodeficiency, the recommended doses are:

  • end-stage renal failure (creatinine clearance less than 10 ml/min) – 800 mg 2 times/day every 12 hours;
  • severe renal failure (creatinine clearance 10-25 ml/min) – 800 mg 3 times/day every 8 hours.

Ointment, Cream

For external use.

Apply 4-6 times/day (every 4 hours) in a thin layer to the affected areas of the skin and areas bordering them.

The drug is applied either with a cotton swab or with clean hands to avoid additional infection of the affected areas.

Therapy should be continued until crusts form on the vesicles, or until they are completely healed.

The duration of therapy is on average 5 days and should not exceed 10 days.

It is important to start treatment of a recurrent infection during the prodromal phase or at the very beginning of the manifestation of the infection.

The duration of treatment is at least 5 days.

In the absence of healing, treatment can be continued for up to 10 days.

If symptoms of the disease persist for more than 10 days, you should consult a doctor.

Lyophilisate

Administered as a slow intravenous infusion.

The dosage regimen is selected depending on the diagnosis, severity of the disease, age and body weight of the patient.

The duration of the course of treatment may vary depending on the location of the infection, the patient’s condition and response to therapy.

Adverse Reactions

The drug is usually well tolerated.

The following classification of adverse reactions was used depending on the frequency of occurrence: very common (>1/10), common (>1/100, <1/10), uncommon (>1/1000, <1/100), rare (>1/10,000, <1/1000), very rare (<1/10,000).

From the digestive system common – nausea, vomiting, diarrhea; very rare – hepatitis, jaundice, in isolated cases – abdominal pain.

From the hematopoietic system uncommon – transient slight increase in liver enzyme activity, slight increase in urea and creatinine concentration, hyperbilirubinemia; very rare – leukopenia, erythropenia, anemia, thrombocytopenia.

From the central nervous system common – headache, dizziness; very rare – agitation, confusion, tremor, ataxia, dysarthria, hallucinations, psychotic symptoms, convulsions, drowsiness, encephalopathy, coma.

These side effects were usually observed in patients with renal failure or in the presence of other provoking factors, and were mainly reversible.

From the respiratory system uncommon – shortness of breath.

Allergic reactions anaphylactic reactions, skin rash, itching, urticaria, Lyell’s syndrome, Stevens-Johnson syndrome.

From the skin and subcutaneous tissue common – itching, urticaria, rash, including sensitization, uncommon – alopecia, rapid diffuse hair loss (since this type of alopecia is observed in various diseases and during therapy with many drugs, its connection with acyclovir intake has not been established); very rare – Lyell’s syndrome, Stevens-Johnson syndrome.

Other common – fatigue, fever; uncommon – peripheral edema, visual impairment, lymphadenopathy, myalgia, malaise.

Contraindications

  • Lactation period;
  • Children under 3 years of age (for this dosage form).

With caution pregnancy; elderly persons and patients taking large doses of acyclovir, especially against the background of dehydration; impaired renal function; neurological disorders or neurological reactions to the administration of cytotoxic drugs (including in the anamnesis).

Use in Pregnancy and Lactation

Acyclovir crosses the placental barrier and accumulates in breast milk.

The use of the drug during pregnancy is possible only if the intended benefit to the mother outweighs the potential risk to the fetus.

If it is necessary to take acyclovir during lactation, breastfeeding should be discontinued.

Use in Renal Impairment

Use with caution in case of impaired renal function.

Caution should be exercised when prescribing acyclovir to patients with renal failure.

In such patients, oral administration of the drug in recommended doses for the treatment and prevention of infections caused by the herpes simplex virus does not lead to the accumulation of the drug to concentrations exceeding established safe levels.

However, in patients with severe renal failure (creatinine clearance less than 10 ml/min), the dose of acyclovir should be reduced to 200 mg 2 times/day with 12-hour intervals.

When treating infections caused by the Varicella zoster virus, Herpes zoster, as well as when treating patients with severe immunodeficiency, the recommended doses are:

Special Precautions

Acyclovir is used strictly as prescribed by a doctor to avoid complications in adults and children over 3 years of age.

Long-term or repeated treatment with acyclovir of patients with reduced immunity may lead to the emergence of virus strains that are insensitive to its action.

Most of the identified acyclovir-insensitive virus strains show a relative lack of viral thymidine kinase; strains with altered thymidine kinase or altered DNA polymerase have been isolated.

In vitro, the effect of acyclovir on isolated strains of Herpes simplex virus can cause the appearance of less sensitive strains.

The drug is prescribed with caution to patients with impaired renal function, elderly patients due to an increase in the half-life of acyclovir.

When using the drug, it is necessary to ensure sufficient fluid intake.

When taking the drug, renal function should be monitored (blood urea concentration and plasma creatinine concentration).

Acyclovir does not prevent the sexual transmission of herpes, so during treatment it is necessary to refrain from sexual intercourse, even in the absence of clinical manifestations.

Patients should be informed about the possibility of transmission of the genital herpes virus during the period of rashes, as well as about cases of asymptomatic virus carriage.

Effect on ability to drive vehicles and mechanisms

No data.

However, it should be borne in mind that during treatment with acyclovir, dizziness may develop, so caution should be exercised when driving vehicles and engaging in potentially hazardous activities that require increased concentration and speed of psychomotor reactions.

Overdose

No cases of overdose with oral administration of acyclovir have been registered.

Oral intake of 20 g of acyclovir has been reported.

Symptoms agitation, coma, convulsions, lethargy.

Precipitation of acyclovir in the renal tubules is possible if its concentration exceeds the solubility in the renal tubules (2.5 mg/ml).

Treatment symptomatic.

Drug Interactions

Simultaneous use with probenecid leads to an increase in the average T1/2 and a decrease in the clearance of acyclovir.

Enhancement of the effect of acyclovir is noted with the simultaneous administration of immunostimulants.

With simultaneous use with nephrotoxic drugs, the risk of impaired renal function increases.

Storage Conditions

Store the drug in a dry, light-protected place at a temperature not exceeding 25°C (77°F). Keep out of reach of children.
