Pharmacological Action
Blocker of histamine H1-receptors. It has antiallergic activity, exerts local anesthetic, antispasmodic, and moderate ganglioblocking action. When taken orally, it causes a sedative and hypnotic effect, exerts a moderate antiemetic effect, and also has central anticholinergic activity.
When administered parenterally to patients with circulating blood volume deficiency, a decrease in blood pressure and an increase in existing hypotension are possible.
When applied topically, it exerts an antiallergic effect.
Pharmacokinetics
It is rapidly absorbed from the gastrointestinal tract. Bioavailability is 50%. Cmax is reached within 20-40 minutes (the highest concentrations are determined in the lungs, spleen, kidneys, liver, brain, and muscles). Plasma protein binding is 98-99%. It penetrates the blood-brain barrier. It is metabolized mainly in the liver, partially in the lungs and kidneys. T1/2 is 4-10 hours. Within 24 hours, it is completely excreted by the kidneys in the form of metabolites conjugated with glucuronic acid. Significant amounts are excreted in breast milk and can cause a sedative effect in infants (a paradoxical reaction characterized by excessive excitability may be observed).
When applied topically, absorption is insignificant; impaired skin integrity increases the absorption of diphenhydramine.
Indications
Allergic conjunctivitis, allergic rhinitis, chronic urticaria, pruritic dermatoses, dermographism, serum sickness; as part of complex therapy for anaphylactic and anaphylactoid reactions, angioedema, and other allergic conditions. Insomnia, chorea, Ménière’s syndrome, seasickness and airsickness, as an antiemetic. Sunburns and first-degree burns; insect bites; skin itching of various origins (except for itching in cholestasis): eczema, chickenpox, allergic skin irritations, contact dermatitis caused by contact with plants.

Dosage Regimen
| The method of application and dosage regimen for a specific drug depend on its form of release and other factors. The optimal dosage regimen is determined by the doctor. It is necessary to strictly adhere to the compliance of the dosage form of a specific drug with the indications for use and dosage regimen. |
Tablets, Solution
It is used orally, parenterally, and topically.
Orally, it is prescribed for adults and children over 14 years of age in a single dose of 50 mg. The regimen and duration of use are determined individually, depending on the indications.
Intravenously or deep intramuscularly, it is prescribed for adults and children over 14 years of age in a single dose of 10-50 mg, the maximum daily dose is 200 mg. For children aged from 7 months to 14 years, the dose is set depending on age.
Topically. Apply to the affected areas of the skin 3-4 times/day.
Adverse Reactions
From the nervous system headache, sedative effect, drowsiness, dizziness, impaired coordination, weakness, confusion, anxiety, agitation, nervousness, tremor, irritability, insomnia, euphoria, paresthesia, neuritis, convulsions.
From the digestive system dryness of the oral mucosa, nausea, vomiting, epigastric pain, anorexia, diarrhea, constipation.
From the respiratory system dryness of the nasal and throat mucosa, increased sputum viscosity, feeling of chest tightness, sneezing, nasal congestion.
From the cardiovascular system palpitations, decreased blood pressure, tachycardia, extrasystole.
From the hematopoietic system hemolytic anemia, thrombocytopenia, agranulocytosis.
From the genitourinary system frequent or difficult urination, urinary retention, early menstruation.
From the sensory organs: visual perception disorder, diplopia, vertigo, tinnitus, acute labyrinthitis.
Allergic reactions: urticaria, skin rash, itching, anaphylactic shock, photosensitivity.
Other : increased sweating, chills.
Contraindications
Hypersensitivity to diphenhydramine.
For systemic use: angle-closure glaucoma, prostatic hypertrophy, stenosing gastric and duodenal ulcer, stenosis of the bladder neck, bronchial asthma, epilepsy; children under 14 years of age (for oral administration); children under 7 months of age (for parenteral administration).
With caution: increased intraocular pressure, hyperthyroidism, arterial hypertension, cardiovascular diseases, bronchopulmonary diseases.
For topical use: simultaneous use of any preparations containing Diphenhydramine; application to large skin surfaces; children under 2 years of age.
With caution: damaged skin at the application site, application to mucous membranes.
Use in Pregnancy and Lactation
During pregnancy and lactation (breastfeeding), Diphenhydramine should be used with caution, strictly according to indications, in cases where the expected therapeutic benefit for the mother outweighs the potential risk to the fetus or breastfed infant.
Significant amounts of diphenhydramine are excreted in breast milk and can cause a sedative effect in infants (a paradoxical reaction characterized by excessive excitability may be observed).
Use in Renal Impairment
It should be used with caution in patients with impaired renal function.
Pediatric Use
It can be used in children according to indications in age-appropriate recommended doses and dosage forms.
Geriatric Use
It should be prescribed with caution to elderly patients to avoid worsening of concomitant diseases.
Special Precautions
During treatment, exposure to solar radiation should be avoided; it is necessary to avoid alcohol consumption.
Effect on ability to drive vehicles and operate machinery
It should be used with caution in patients engaged in potentially hazardous activities requiring increased attention and rapid psychomotor reactions.
Drug Interactions
Concomitant use enhances the effect of ethanol and drugs that depress the central nervous system.
Concomitant use of MAO inhibitors enhances the anticholinergic activity of diphenhydramine.
Antagonistic interaction is noted with simultaneous administration with psychostimulants.
It reduces the effectiveness of apomorphine as an emetic in the treatment of poisoning.
It enhances the anticholinergic effects of drugs with anticholinergic activity.
Storage Conditions
Store at 2°C (36°F) to 25°C (77°F). Keep in original packaging, protected from light. Keep out of reach of children.
