Pharmacological Action

Agent for rehydration and detoxification, for correction of hypoglycemia.

An isotonic dextrose solution (5%) is used to replenish body fluid. It is also a source of a valuable nutrient that is easily absorbed. During the metabolism of glucose in tissues, a significant amount of energy necessary for the body’s vital functions is released.

When hypertonic solutions (10%, 20%, 40%) are administered intravenously, the osmotic pressure of the blood increases, the flow of fluid from tissues into the blood is enhanced, metabolic processes are intensified, the antitoxic function of the liver is improved, the contractile activity of the heart muscle is enhanced, blood vessels dilate, and diuresis increases.

Pharmacokinetics

During the administration of the solution, dextrose first enters the intravascular space and then passes into the cells.

During glycolysis, glucose is converted to pyruvate or lactate. Lactate can then be partially re-involved in glucose metabolism through the reactions of the Cori cycle. Pyruvate is completely oxidized by oxygen to CO2 and H2O. The oxidation products of glucose are excreted by the lungs (CO2) and kidneys (H2O).

Normally, glucose is not eliminated by the kidneys. In pathological conditions, such as diabetes mellitus, metabolic disorders with hyperglycemia, when the blood glucose concentration exceeds 180 mg/100 ml or 10 mmol/l, glucose is excreted by the kidneys (glycosuria).

Indications

Compensation for carbohydrate deficiency in the body. Correction of dehydration due to vomiting, diarrhea, in the postoperative period. Detoxification infusion therapy. Collapse, shock (as a component of various blood-substituting and anti-shock fluids). Used for preparing solutions of medicinal products for intravenous administration.

Dosage Regimen

The method of application and dosage regimen for a specific drug depend on its form of release and other factors. The optimal dosage regimen is determined by the doctor. It is necessary to strictly adhere to the compliance of the dosage form of a specific drug with the indications for use and dosage regimen.

Tablets, Solution

Intravenously, by drip.

5% solution: maximum up to 150 drops/min, maximum daily dose for adults – 2 L;

10% solution: maximum up to 60 drops/min, maximum daily dose for adults – 500 mL;

20% solution: maximum up to 40 drops/min, maximum daily dose for adults – 300 mL;

40% solution: maximum up to 30 drops/min, maximum daily dose for adults – 250 mL.

Orally.

Adverse Reactions

From the metabolism possible electrolyte imbalance, hyperglycemia.

From the cardiovascular system hypervolemia, acute left ventricular failure.

Other fever.

Local reactions rarely – local irritation, infection development, thrombophlebitis.

Contraindications

Hyperglycemia, diabetes mellitus, hyperlactacidemia, hyperhydration, postoperative disorders of glucose utilization; circulatory disorders threatening cerebral and pulmonary edema; cerebral edema, pulmonary edema, acute left ventricular failure, hyperosmolar coma.

When large volumes are necessary, contraindications related to the dose of dextrose and/or volume overload may manifest: hyperhydration; hypotonic dehydration.

Use in Pregnancy and Lactation

Dextrose can be used during pregnancy and breastfeeding according to indications. Blood glucose concentration should be monitored.

Use in Renal Impairment

Dextrose should be used with caution in chronic renal failure (oligoanuria).

Pediatric Use

Can be used in children according to indications in age-appropriate recommended doses.

Special Precautions

Dextrose should be used with caution in decompensated heart failure, chronic renal failure (oligoanuria), hyponatremia.

To increase osmolarity, a 5% dextrose solution can be combined with an isotonic sodium chloride solution.

Storage Conditions

Store at 2°C (36°F) to 25°C (77°F). Keep in original packaging, protected from light. Keep out of reach of children.
