Pharmacological Action
It participates in various metabolic processes in the body. Infusion of dextrose solutions partially replenishes water deficit.
Dextrose, upon entering the tissues, is phosphorylated, converting into glucose-6-phosphate, which is actively involved in many links of the body’s metabolism. A 5% dextrose solution is isotonic to blood plasma.
Hypertonic solutions (10%, 20%, 40%) increase the osmotic pressure of the blood and increase diuresis. The theoretical osmolarity of 10% dextrose is 555 mOsm/l, 20% – 1110 mOsm/l.
Pharmacokinetics
It is completely absorbed by the body, not excreted by the kidneys (its appearance in urine is a pathological sign).

- Intoxication in liver diseases (hepatitis, dystrophy and liver atrophy, including hepatic failure);

Dosage Regimen
| The method of application and dosage regimen for a specific drug depend on its form of release and other factors. The optimal dosage regimen is determined by the doctor. It is necessary to strictly adhere to the compliance of the dosage form of a specific drug with the indications for use and dosage regimen. |
Solution
Intravenously by drip.
A 5% solution is administered at a maximum rate of up to 7 ml (150 drops)/min (400 ml/h); the maximum daily dose for adults is 2 L.
A 10% solution – up to 60 drops/min (3 ml/min); the maximum daily dose for adults is 1 L.
A 20% solution – up to 30-40 drops/min (1.5-2 ml/min); the maximum daily dose for adults is 500 ml.
A 40% solution – up to 30 drops/min (1.5 ml/min); the maximum daily dose for adults is 250 ml.
Intravenously by bolus.
10-50 ml of 5% and 10% solutions.
In adults with normal metabolism, the daily dose of administered glucose should not exceed 4-6 g/kg, i.e., about 250-450 g (with a decrease in metabolic rate, the daily dose is reduced to 200-300 g), while the volume of fluid administered is 30-40 ml/kg.
For children for parenteral nutrition, on the first day, 6 g of glucose/kg/day is administered, subsequently – up to 15 g/kg/day, along with glucose, fats and amino acids are administered if necessary.
When calculating the glucose dose when administering 5% and 10% solutions, the permissible volume of fluid administered should be taken into account: for children weighing 2-10 kg – 100-165 ml/kg/day, for children weighing 10-40 kg – 45-100 ml/kg/day.
With normal metabolic status, the maximum rate of glucose administration for adults is 0.25-0.5 g/kg/h (with a decrease in metabolic rate, the administration rate is reduced to 0.125-0.25 g/kg/h). In children, solutions with a concentration not exceeding 20-25% are used; the rate of glucose administration should not exceed 0.75 g/kg/h.
Tablets, Powder
IV drip.
5% solution: maximum up to 150 drops/min, maximum daily dose for adults – 2 L;
10% solution: maximum up to 60 drops/min, maximum daily dose for adults – 500 ml;
20% solution: maximum up to 40 drops/min, maximum daily dose for adults – 300 ml;
40% solution: maximum up to 30 drops/min, maximum daily dose for adults – 250 ml.
Orally.
Adverse Reactions
Hypervolemia, acute left ventricular failure; if the infusion rate is exceeded – osmotic diuresis with loss of water, electrolytes and development of hyperosmolar coma. At the injection site – development of infection, thrombophlebitis.
Contraindications
- Hyperglycemia;
- Hyperlactacidemia;
- Hyperhydration;
- Postoperative disorders of glucose utilization;
- Circulatory disorders threatening cerebral and pulmonary edema;
- Cerebral edema;
- Pulmonary edema;
- Acute left ventricular failure;
- Hyperosmolar coma;
- Childhood (for solutions over 20-25%);
- Hypersensitivity.
With caution decompensated chronic heart failure, chronic renal failure (oligo-, anuria), hyponatremia, diabetes mellitus.
Use in Pregnancy and Lactation
There are no data on the negative effect of the drug during the specified period.
Use in Renal Impairment
With caution: chronic renal failure.
Pediatric Use
Contraindicated in childhood (for solutions over 20-25%).
Special Precautions
Monitoring of water-electrolyte balance.
To increase osmolarity, a 5% solution can be combined with an isotonic sodium chloride solution.
For more complete and rapid absorption of glucose administered in large doses, 4-5 units of short-acting insulin can be administered subcutaneously simultaneously with it, based on 1 unit of insulin per 4-5 g of glucose.
In patients with diabetes mellitus, glucose is administered under the control of its content in the blood and urine.
Effect on ability to drive vehicles and operate machinery
No negative impact on the ability to drive vehicles and work with machinery has been identified.
Overdose
Symptoms hyperglycemia, glucosuria, hyperglycemic, hyperosmolar coma, hyperhydration, disturbance of water-electrolyte balance, increased liponeogenesis with increased production of CO2, which leads to a sharp increase in minute respiratory volume and, as a consequence, respiratory rate, fatty infiltration of the liver.
Treatment discontinue glucose administration, administer short-acting insulin, symptomatic therapy.
Drug Interactions
When combined with other drugs, it is necessary to monitor pharmaceutical compatibility, including visually.
Storage Conditions
At a temperature from 5 to 30°C (86°F). Keep out of reach of children.
