Pharmacological Action

A means for rehydration and detoxification, it restores the water and electrolyte composition of the blood. When used as a means for replenishing the circulating blood volume (CBV), due to its rapid exit from the bloodstream into the extravascular space, the effect lasts only for 30-40 minutes (which is why the solution is only suitable for short-term replenishment of CBV).

The main cation of the extracellular fluid, sodium, is involved in controlling water distribution, water balance, and the osmotic pressure of body fluids. It is also associated with chloride and bicarbonate in regulating the body’s acid-base balance.

The main cation of the intracellular fluid, potassium, is involved in the synthesis of carbohydrates and proteins, nerve impulse conduction, and muscle contraction.

Calcium in ionized form is necessary for the functional mechanism of blood coagulation, normal heart function, and the regulation of neuromuscular excitability.

The main extracellular anion, chloride, is closely related to sodium metabolism and is involved in regulating the body’s acid-base balance.

Pharmacokinetics

The pharmacokinetics of sodium and chloride ions are the same as those from dietary intake. They are distributed to all organs, tissues, and interstitial spaces and are excreted by the kidneys via glomerular filtration. Significant reabsorption of Na+ and Cl ions occurs in the tubules, especially in the loop of Henle and distal tubules.

Potassium ions are freely filtered in the glomeruli but are almost completely reabsorbed in the proximal tubules (excretion is about 100%). The kidneys have a limited ability to conserve K+ concentration. Therefore, when the Na+ concentration in the distal tubules is high, the loss of K+ can be significant, and hypokalemia may develop. This justifies the presence of K+ in the composition of the product.

The homeostasis of calcium ions is well controlled by hormones and rarely requires clinical intervention with intravenous infusion of the solution.

The product is not metabolized in the body and is rapidly excreted by the kidneys (80% within 4 hours, completely excreted within 12-24 hours).

Indications

Dehydration and electrolyte imbalance (3rd and 4th-degree thermal burns), acute blood loss, correction of water-salt balance in acute diffuse peritonitis and intestinal obstruction, acute intestinal infections (severe course, inability to take oral rehydration agents), hypovolemic shock, therapeutic plasmapheresis, intestinal fistula.

Dosage Regimen

The method of application and dosage regimen for a specific drug depend on its form of release and other factors. The optimal dosage regimen is determined by the doctor. It is necessary to strictly adhere to the compliance of the dosage form of a specific drug with the indications for use and dosage regimen.

Solution

Intravenously, by drip, at a rate of 60-80 drops/min, or by stream. The daily dose for adults is 5-20 ml/kg, which can be increased to 30-50 ml/kg if necessary. The daily dose for children is 5-10 ml/kg, the rate of administration is 30-60 drops/min; in shock, 20-30 ml/kg is initially administered. The course of treatment is 3-5 days. For therapeutic plasmapheresis, it is administered in a volume twice the volume of removed plasma (1.2-2.4 L), in case of severe hypovolemia – in combination with colloid solutions. The maximum volume of the administered solution is 3 L/day.

Adverse Reactions

Possible electrolyte imbalance, hyperhydration, allergic reactions, fever (with rapid administration), local reactions at the injection site.

Contraindications

Hypersensitivity to the components of the product; hypernatremia, hyperchloremia, hypercalcemia, hyperkalemia, acidosis, chronic heart failure NYHA functional class III-IV, pulmonary edema, cerebral edema, hypercoagulation, hypervolemia, thrombophlebitis, metabolic alkalosis, severe chronic renal failure with oligo- and anuria, concomitant therapy with corticosteroids.

With caution: arterial hypertension; cardiovascular diseases (including chronic heart failure NYHA functional class I-II), simultaneous use of cardiac glycosides, simultaneous administration with blood products due to the risk of coagulation, hepatic insufficiency; preeclampsia; peripheral edema of various origins; hyperaldosteronism and other pathologies associated with hypernatremia or hyperkalemia (mild to moderate renal failure, adrenal insufficiency, acute dehydration, extensive tissue breakdown); diseases and conditions predisposing to increased vitamin D concentration (including sarcoidosis).

Use in Pregnancy and Lactation

During pregnancy, it can be used according to indications in cases where the expected benefit to the mother outweighs the potential risk to the fetus.

If it is necessary to use during lactation, the issue of discontinuing breastfeeding should be decided.

Use in Hepatic Impairment

Use with caution in hepatic insufficiency.

Use in Renal Impairment

Contraindicated in severe chronic renal failure with oligo- and anuria. Use with caution in patients with kidney diseases.

Pediatric Use

Can be used in children according to indications in doses recommended for the respective age.

Special Precautions

During long-term therapy, it is necessary to determine laboratory parameters together with an assessment of the patient’s condition to monitor water-electrolyte balance and acid-base balance every 6 hours (depending on the infusion rate).

In case of rapid administration of a large volume of the solution, it is necessary to monitor the acid-base status and electrolyte concentrations. A decrease in blood pH leads to an increase in the K+ content in the blood serum.

The use of this product may cause fluid volume overload, congestion, and pulmonary edema. The risk of dilution is inversely proportional to the electrolyte concentration, and the risk of volume overload is directly proportional.

Due to the content of sodium ions, this product should be used with caution in patients with clinical conditions accompanied by sodium retention and edema, as well as in those receiving corticosteroids or corticotropin.

Due to the content of potassium ions, this product should be used with caution in patients with cardiovascular diseases, hyperkalemia, renal failure, and clinical conditions accompanied by potassium retention.

Due to the high content of chloride ions, long-term use of the product is not recommended.

Due to the content of calcium ions, this product should be used with caution in patients taking cardiac glycosides. ECG monitoring is necessary. The concentration of calcium in blood plasma does not always correlate with the concentration of calcium in tissues.

Drug Interactions

When combined with any other drugs, it is necessary to visually control pharmaceutical compatibility.

Concomitant use with NSAIDs, androgens, anabolic hormones, estrogens, corticotropin, mineralocorticoids, vasodilators, or ganglion blockers may increase sodium retention in the body.

Concomitant use with potassium-sparing diuretics, ACE inhibitors, angiotensin II receptor antagonists, tacrolimus, cyclosporine, and potassium preparations increases the risk of hyperkalemia.

Concomitant use with thiazide diuretics or vitamin D increases the risk of hypercalcemia.

In combination with cardiac glycosides, the likelihood of toxic effects of the latter increases.
