Pharmacological Action
Osmotic diuretic. By increasing plasma osmotic pressure and filtration without subsequent tubular reabsorption, it leads to water retention in the tubules and an increase in urine volume.
By increasing plasma osmolarity, it causes fluid movement from tissues (in particular, the eyeball, brain) into the vascular bed. It causes a pronounced diuretic effect, which is characterized by the excretion of a large amount of osmotically free water, as well as sodium and chloride, without significant excretion of potassium.
It causes an increase in circulating blood volume.
Pharmacokinetics
After IV administration, the Vd of mannitol corresponds to the volume of extracellular fluid. Mannitol may undergo minor metabolism in the liver to form glycogen.
Mannitol excretion is regulated by glomerular filtration without significant tubular reabsorption. T1/2 is about 100 min. It is excreted by the kidneys; after IV administration of a 100 g dose, 80% is detected in urine within 3 hours.
In renal failure, T1/2 may increase to 36 hours.
Indications
Cerebral edema. Intracranial hypertension. Status epilepticus. Intraocular hypertension, acute attack of glaucoma.
Oliguria in acute renal failure. For determining the glomerular filtration rate in acute oliguria.
Acute hepatic failure. Acute hepatic failure in patients with preserved renal filtration capacity and other conditions requiring increased diuresis.
Poisoning with barbiturates, salicylates, bromides, lithium preparations, forced diuresis in other poisonings.
Post-transfusion complications after administration of incompatible blood.
For the prevention of hemolysis and hemoglobinemia during transurethral resection of the prostate or during surgical procedures such as cardiopulmonary bypass, during operations with extracorporeal circulation.

Dosage Regimen

Solution
Mannitol is administered IV or by inhalation in appropriate dosage forms.
For IV administration (slow bolus or drip), the prophylactic dose is 500 mg/kg body weight, the therapeutic dose is 1-1.5 g/kg. The daily dose should not exceed 140-180 g. For operations with extracorporeal circulation, it is administered at a dose of 20-40 g immediately before surgery.
Patients with oliguria should first be given an IV test dose of mannitol (200 mg/kg) over 3-5 minutes. If no increase in urine output to 30-50 ml/hour is observed within 2-3 hours after this, further administration of mannitol should be avoided.
Adverse Reactions
Metabolism: disorders of water-electrolyte balance (increased circulating blood volume, dilutional hyponatremia, hyperkalemia) and their manifestations (muscle weakness, cramps, dry mouth, thirst, impaired consciousness).
Other: tachycardia, chest pain, thrombophlebitis, skin rash.
Contraindications
Chronic renal failure, impaired renal filtration function, left ventricular failure (especially accompanied by pulmonary edema), hemorrhagic stroke, subarachnoid hemorrhage (except for bleeding during craniotomy), severe forms of dehydration, hyponatremia, hypochloremia, hypokalemia, hypersensitivity to mannitol.
Use in Pregnancy and Lactation
Adequate and strictly controlled studies in humans have not been conducted.
The use of mannitol during pregnancy and lactation is possible in cases where the intended benefit to the mother outweighs the potential risk to the fetus or breastfed infant.
Use in Hepatic Impairment
Used according to indications.
Use in Renal Impairment
Contraindicated in chronic renal failure, impaired renal filtration function.
Use with caution in patients with renal impairment.
Pediatric Use
It is not recommended to use Mannitol by inhalation in children under 6 years of age due to insufficient data on safety and efficacy.
Geriatric Use
Use with caution to avoid the risk of exacerbation of chronic diseases.
Special Precautions
Use with caution in patients with severe forms of chronic heart failure, hypovolemia, renal impairment.
If symptoms such as headache, vomiting, dizziness, visual disturbances appear during mannitol administration, the infusion should be stopped and the development of complications such as subdural and subarachnoid bleeding should be ruled out.
When using mannitol, it is necessary to monitor blood pressure, diuresis, and serum electrolyte concentrations (potassium, sodium).
Drug Interactions
With simultaneous use of mannitol with cardiac glycosides, an increase in their toxic effect associated with hypokalemia is possible.
